二维超声成像在急性心肌梗死患者左室功能评估中的应用(1)

     [摘要] 目的 探讨二维超声成像在评估急性心肌梗死患者左室功能变化情况。 方法 回顾性分析我院2009年10月~2011年5月收治入院的40例急性心梗患者临床资料,根据梗阻部位不同分为两组,前壁心梗组(20例)和下壁心梗组(20例),两组均采用二维超声斑点追踪成像(STI)对两组心肌梗死患者行左心室各节段圆周应变峰值测定。 结果 常规超声检查中前壁心梗组左室舒张末期内径(LVDD)、左室收缩末期内径(LVDS)明显大于下壁心梗组,左心室射血分数(LVEF)明显小于下壁心梗组。前壁心梗组前壁、室间隔、心尖各节段圆周应变峰值较下壁心梗组明显减小,而下壁心梗组后壁、下壁各节段圆周应变峰值较前壁心梗组减小(P < 0.05);且前壁心梗组左心室整体圆周应变(GCS)较下壁心梗组减小(P < 0.01)。 结论 二维超声成像能够定量评价急性心肌梗死患者左室功能变化情况,为临床评价患者的左心功能变化提供了一个有效、无创的方法。

    Application of two-dimensional ultrasound imaging in the evaluation of left entricular function of patients with acute myocardial infarction

    [Abstract] Objective To study the changes of two-dimensional ultrasound imaging in the evaluation of left entricular function of patients with acute myocardial infarction. Methods The clinical data of 40 patients with acute myocardial infarction in our hospital from October 2009 to May 2011 were analyzed retrospectively. All patients were divided into two groups, anterior wall myocardial infarction group (20 cases) and inferior myocardial infarction group (20 cases) according to different parts of obstruction. Each segment of circumferential strain of patients with left ventricular myocardial infarction of the two groups was detected by two-dimensional ultrasound spots tracking imaging (STI). Results In the conventional ultrasound examination, the LVDD, LVDS of anterior wall myocardial infarction group were significantly greater than the inferior myocardial infarction group, LVEF was obviously less than the inferior myocardial infarction group. Each segment of circumferential strain of anterior wall, interventricular septum and cardiac apex in the anterior wall myocardial infarction group was significantly less than the inferior myocardial infarction group, while each segment of circumferential strain of posterior wall and inferior wall was less than the inferior myocardial infarction group (P < 0 ......
