人乳头状瘤病毒E6、E7蛋白在维吾尔族及汉族宫颈癌患者中的表达及意义(1)

     [摘要] 目的 分析维吾尔族及汉族宫颈癌患者中人乳头状瘤病毒(HPV)E6、E7蛋白表达与宫颈癌癌前病变恶性进展的关系,进一步了解其在维吾尔族及汉族宫颈癌患者间是否存在差异。 方法 选取2013年4月~2014年4月在新疆维吾尔自治区人民医院妇科门诊行第二代杂交捕技术HPV DNA检测结果为阳性的133例维吾尔族(n=75)和汉族(n=58)患者宫颈不同病变程度的石蜡包埋宫颈组织。采用免疫组化法分别检测各组织中HPV E6、E7蛋白表达,使用Spearman等级相关分析HPV E6、E7蛋白表达与病变程度的相关性,并采用χ2检测比较维吾尔族及汉族宫颈癌患者HPV E6、E7蛋白表达的差异。 结果 维吾尔族和汉族慢性宫颈炎、CIN 1、CIN 2~3、宫颈鳞癌组织中HPV E6的阳性率,随着病情的进展呈逐渐增高趋势,差异均有统计学意义(χ2=39.61,P=0.001;χ2=19.77,P=0.001);Spearman等级相关分析发现,维吾尔族及汉族宫颈癌患者HPV E6蛋白的表达与宫颈病变级别均呈显著正相关(r=0.708,P=0.001;r=0.729,P=0.001)、HPV E7在维吾尔族和汉族慢性宫颈炎、CIN 1、CIN 2~3、宫颈鳞癌组织中的阳性表达率,随着病情的进展呈逐渐增高,多组比较差异有统计学意义(χ2=32.48,P=0.001;χ2=24.16,P=0.001);Spearman等级相关分析发现,维吾尔族及汉族宫颈癌患者E6蛋白表达与宫颈病变级别均呈显著正相关(r=0.649,P=0.001;r=0.759,P=0.001);慢性宫颈炎、CIN 1、CIN 2~3维吾尔族患者HPV E6、E7蛋白的阳性表达率高于汉族,维吾尔族宫颈癌患者HPV E6、E7蛋白表达率低于汉族,但差异均无统计学意义(P > 0.05)。 结论 HPV E6、E7蛋白是维吾尔族和汉族宫颈癌发展的重要因素,HPV E6、E7蛋白可能具有预测宫颈病变进展的价值。

    [Abstract] Objective To analyze the relation between the expression of HPV E6/E7 gene and cervical cancer progression in Uyghur and Han women and to compare the difference expression of HPV E6/E7 gene between Uygur and Han women. Methods Total of 133 paraffin-embedded tissues of cervix were collected from the People's Hospital of Xinjiang Uygur Autonomous Region from April 2013 to April 2014 were recruited, and all the cases were confirmed by the histological diagnosis. Immuno-histochemistry was performed to detect the expressions of HPV16 E6 and E7 proteins including 75 Uygur and 58 Han. The correlation between expression of HPV E6, E7 protein and lesion grade of cervical was analyzed by spearman rank correlation, and to compare the difference of HPV E6, E7 protein expression between Chinese and Uygur by χ2 trest. Results The expression rate of HPV E6 proteins were increased with the development of from chronic inflammation to caner in Uygur and Han women, respectively, there were significant difference(χ2=39.61, P=0.001; χ2=19.77, P=0.001); further using Spearman rank correlation found the significant positive correlation between expression of E7 proteins and lesion grade of cervical (r=0.708, P=0.001; r=0.729, P=0.001).The expression rate of HPV E7 proteins were increased with the disease development from chronic inflammation to caner in Uygur and Han women, respectively, there were significant differences (χ2=32.48, P=0.001; χ2=24.16, P=0.001), further using Spearman rank correlation found significant positive correlation between expression of E7 proteins and lesion grade of cervical (r=0.649, P=0.001; r=0.759, P=0.001).In the inflammatory group, CIN 1, CIN 2-3, positive expression rate of E6, E7 protein in Uygur women was higher than that of Han women; in cervical cancer group, the expression rate of E6, E7 protein in Uygur women was lower than that of Han women, but the difference was not statistically significant (P > 0.05). Conclusion HPV E6, E7 protein were important factors in the development of cervical cancer in Uygur and Han women. HPV E6, E7 proteins may have a prediction value in development of cervical lesion.
