双能X线吸收测量法在单纯性肥胖症减肥效果监测中的应用(1)

     [摘要] 目的 研究运用双能X线吸收测量法(DEXA)测量肥胖症患者减肥效果,为减肥治疗提供科学客观的评价指标。 方法 选取2014年8月~2015年6月佛山市中医院就诊的80例单纯性肥胖症患者为研究对象,按照性别不同,将患者分为男性组(45例)和女性组(35例)。分别采用DEXA对两组患者减肥治疗前后进行全身组织成分含量测定,包括体内脂肪含量(BF%)、脂肪量指数(FMI)及腹部内脏脂肪量面积(VAT);检测比较两组治疗前后体重指数(BMI)、低密度脂蛋白-胆固醇(LDL-C)、三酰甘油(TG)、总胆固醇(TC)情况;分析BF%、FMI、VAT与BMI及LDL-C、TG、TC的变化量与VAT的相关性。 结果 两组患者治疗后的BF%、FMI、VAT及BMI均较治疗前明显降低(P < 0.05或P < 0.01),且变动方向与BMI相一致;治疗前后两组患者的BF%、FMI、VAT均与BMI呈显著正相关(P < 0.01);治疗后两组患者的LDL-C、TG、TC与治疗前比较,均明显降低(P < 0.05或P < 0.01),且两组LDL-C、TG、TC的变化量与VAT的变化量均呈显著正相关(P < 0.01)。 结论 DEXA法测定的指标与BMI的评价一致性较好,可作为监测肥胖症减肥效果的评价指标,尤其是VAT。

    [Abstract] Objective To study on the observe reduce weight of simple obesitys with dual energy X-ray absorptiometry method (DEXA), pvovide scientific and obiective evaluating indicator for cure of obesitys. Methods 80 cases of simple obesitys patients as research subjects were selected from august 2014 to June 2015 in the Traditional Chinese Medicine Hospital of Foshan City. Patients were divided into male group (45 cases) and female group (35 cases) according to gender, and the ingredients of full body were measured by DEXA before and after treatment, including the body fat content (BF%), fat mass index (FMI) and visceral fat area (VAT); the BMI, LDL-C, TG, TC of patients in two groups were also detected and compared before and after treatment; the correlation between BF%, FMI, VAT and BMI, the variable quantity of TC, TG, LDL-C and variable quantity of VAT were analyzed. Results After treatment, BF%, FMI, VAT and BMI of two groups were significantly lower than before treatment (P < 0.05 or P < 0.01), and the change direction of these indexes were coincided with BMI. The BF%, FMI and VAT of two groups were significantly correlated with BMI before and after treatment (P < 0.01). LDL-C, TG, TC of two groups after treatment were significantly lower than before treatment (P < 0.05 or P < 0.01), and the variable quantity of TC, TG, LDL-C of two groups were significantly correlated with variable quantity of VAT (P < 0.01). Conclusion Determination indexes of DEXA are good agreement with BMI, can be used to monitor the effect of reduce weight, especial for VAT.

    随着国人生活水平的提高,肥胖症发生率有加速增高的趋势。肥胖症常伴发糖尿病、高血压病、冠心病等多种慢性疾病,是严重影响人们健康和生活质量的疾病之一,已成为世界各国面临的严重公共健康问题。适量的体重减轻能明显改善肥胖症患者的生活质量和降低相关并发症的死亡率。体重指数(BMI)测量法是WHO国际通用的肥胖分型法,其具有操作简便的优势,但该法也具有不能测定局部脂肪分布与脂肪含量的劣势,即不能通过测定脂肪百分率(BF%)、脂肪指数(FMI)、腹部内脏脂肪面积(VAT)等指标来更加准确地反映患者的肥胖程度。故为了探究更加准确、科学的肥胖症诊断法,以便准确测量患者的局部脂肪分布与脂肪含量,本研究采用双能X线吸收测量法(DXA Dual energy X-ray absorptiometry,DEXA)定量监测肥胖症患者减肥过程的变化,期待为临床评价减肥治疗效果提供科学客观的指标。 (黎慧萍 余寿益 刘继洪 梁绮君)
