芪苈强心胶囊联合托拉塞米片治疗老年原发性高血压合并心力衰竭的效果及安全性(1)
[摘要] 目的 探討芪苈强心胶囊联合托拉塞米片治疗老年原发性高血压合并心力衰竭的临床效果。 方法 选择2015年3月～2016年3月于延安大学附属医院老年病科接受治疗的78例老年原发性高血压合并心力衰竭患者作为研究对象，将其随机分为观察组和对照组，每组39例。对照组给予托拉塞米片治疗，观察组在对照组治疗的基础上给予芪苈强心胶囊治疗，治疗时间均为6个月。治疗后对两组疗效进行评价。治疗前后分别检测两组患者收缩压、舒张压及24 h血压变异等血压指标水平；左室舒张末内径(LVEDD)、左心室射血分数(LVEF)、左室收缩末内径(LVESD)、左房内径(LADD)及左室舒张早期与晚期充盈速度比值(E/A)等心脏功能指标水平；心率、心胸比、6 min步行试验(6WMT)及脑钠肽前体(NT-proBNP)等一般心功能指标水平。观察所有患者治疗期间出现的不良反应。 结果 观察组总有效率为94.9%，明显高于对照组(82.1%)(P < 0.05)。治疗后，两组患者血压指标、心脏功能指标及一般心功能指标水平均明显得到改善，且观察组改善程度明显优于对照组，差异均有统计学意义(均P < 0.05)。所有患者均未出现严重药物不良反应。 结论 芪苈强心胶囊联合托拉塞米片治疗老年原发性高血压合并心力衰竭效果较好，值得进行深入研究。

[Abstract] Objective To investigate the clinical efficacy of Qili Qiangxin Capsules combined with Torasemide Tablets in the treatment of primary hypertension with heart failure in elderly patients. The levels of blood pressure indexes (systolic pressure， diastolic pressure and 24 h blood pressure variation)， cardiac function indexes [left ventricular end-diastolic dimension (LVEDD)， left ventricular ejection fraction (LVEF)， left ventricular end systolic diameter (LVESD)， left anterior descending diagonal (LADD) and left ventricular early and late diastolic filling velocity ratio (E/A)]， general cardiovascular indexes [cardiothoracic ratio， 6WMT， pro-brain natriuretic peptide (NT-proBNP) content and heart rate] before and after treatment were detected. Results The total effective rate of observation group were 94.9%， which was higher than that of control group (82.1%) (P < 0.05). After treatment， the blood pressure indexes， cardiac function indexes， general cardiovascular indexes in the two groups were all significantly improved and the improvement degree of observation group was better than that of control group， the differences were all statistically significant (all P < 0.05). All patients had no serious adverse drug reactions. Conclusion Qili Qiangxin Capsules combined with Torasemide Tablets in the treatment of primary hypertension with heart failure in elderly patients has a good clinical efficacy and it is worthy of further study., 百拇医药(任阳 谢圆媛)
