超声乳化联合人工晶体植入术对硬核白内障患者视力改善及视觉相关生活质量的影响(1)
[摘要] 目的 觀察硬核白内障患者行超声乳化联合人工晶体植入术前后视觉相关生活质量的变化。 方法 回顾性病例自身对照研究。选取2014年6月～2016年4月于湖州师范学院附属第一医院就诊并行白内障超声乳化联合人工晶状体植入术治疗的Ⅲ～Ⅴ级硬核白内障患者60例(60眼)，评估患者术后角膜水肿的情况，并采用中文版25项美国国家眼科研究所视觉功能问卷调查(NEI-VFQ-25)和标准对数视力表对患者术前及术后3个月时的logMAR视力及视觉相关的生活质量进行评价分析。 结果 ①60例硬核白内障患者均成功行超声乳化联合人工晶体植入术。40例硬核白内障患者出现角膜水肿，轻度水肿21眼，5 d内消退；中度水肿13眼，1周内消退；重度水肿6眼，14～21 d消退。②硬核白内障患者术后3个月的远视力(0.32±0.13)和近视力(0.56±0.21)，与术前的远视力(1.12±0.38)和近视力(1.02±0.32)比较，差异有统计学意义(均P < 0.01)。③硬核白内障患者术后3个月的视觉相关生活质量，包括一般健康状态、总体视觉情况、近距离工作、远距离工作、社交功能、精神健康状态、社会角色限制、独立性、驾驶、色彩视觉及周边视野等方面评分，与术前比较，差异有统计学意义(均P < 0.01)，而术后3个月时眼痛指标评分与术前比较，差异无统计学意义(P > 0.05)。 结论 白内障超声乳化摘除联合人工晶状体植入术能够明显改善硬核白内障患者的远近视力及视觉相关的生活质量。

[Abstract] Objective To evaluate the influence of phacoemulsification on the visual function and vision-related life quality in patients with hard nuclear cataract. Methods Retrospective case-control study. 60 patients (60 eyes) with hard nuclear cataract who received treatment in the First Affiliated Hospital of Huzhou University were included in this study from June 2014 to April 2016. The corneal edema on hard nuclear cataract was observed after surgery. Visual acuities， and vision-related life quality before surgery and 3 months later were performed with application of the standard logarithmic eye chart and 25-item National Eye Institute Visual Function Questionnaire. Results ①60 patients with hard nuclear cataract were successfully performed phacoemulsification and intraocular lens of all eyes were implanted successfully. 40 eyes developed corneal edema in different degrees. Among them， 21 eyes had mild level corneal edema and recovered to transparency within 5 d. 13 eyes had moderate level corneal edema and recovered to transparency within 1 week. 6 eyes had severe level corneal edema and recovered to transparency within 14-21 d. ②The distance vision and near vision were 1.12±0.38 and 1.02±0.32 in preoperation， 0.32±0.13 and 0.56±0.21 in postoperation， respectively， showed significant enhancements in distance vision and near vision after operation (all P < 0.01). ③Except the degree of eye pain， the indicators from NEI-VFQ25 table including general health status， overall vision， near activities， distant activities， social functional， mental health， social role difficulties， independency， driving， color vision， perimetry were obviously improved 3 months after phacoemulsification and intraocular lens implantation surgery. Conclusion The phacoemulsification and intraocular lens implantation surgery can greatly increase the visual function and the vision-related quality of life in patients with hard nuclear cataract., 百拇医药(卢琪芳 杨卫华)
