教师标准化病人在妇产科教学实践中的应用(1)

     [摘要] 目的 探讨教师标准化病人(TSP)在妇产科学临床教学实践中的应用价值。 方法 选择在新疆医科大学第三临床学院见习的2013级五年制临床医学专业115名学生为研究对象,将其随机分为两组,实验组60名,对照组55名,分别进行TSP教学和常规教学。教学结束后对两组学生进行理论及临床技能考核;同时,就教学情况对两组学生进行教学满意度调查。 结果 实验组学生对教学的满意度高于对照组,实验组学生的理论成绩明显高于对照组,差异有统计学意义(P < 0.05);两组学生的书写完整病历成绩差异无统计学意义(P > 0.05);实验组学生病史采集、妇科检查、诊断依据、鉴别诊断、医患沟通的成绩高于对照组(P < 0.05);在制订诊疗计划及接诊态度方面,两组学生成绩差异无统计学意义(P > 0.05)。 结论 在妇产科学临床教学中,应用TSP可以提高医学生的临床实践技能。

    [Abstract] Objective To explore the application value of teachers standardized patients (TSP) in clinical teaching practice of obstetrics and gynecology. Methods A total of 115 students from the five-year clinical medicine specialty of 2013 in the Third Clinical Medical College of Xinjiang Medical University were selected as research objects and randomly divided into two groups: experimental group (60 students) and control group (55 students). They were taken TSP teaching and regular teaching respectively. At the end of the teaching, the theoretical and clinical skills in the two groups were assessed; at the same time, the teaching satisfaction of the two groups for the teaching situation was surveyed. Results The satisfactory rate of the students in the experimental group was significantly higher than that in the control group, the theoretical scores of the students in the experimental group were significantly higher than those in the control group, the differences were all statistically significant (P < 0.05). There was no statistically significant difference of the scores of writing complete medical records between the two groups (P > 0.05). The scores of history-taking, gynecologic examination, diagnostic basis, differential diagnosis, doctor-patient communication in the students of experimental group were higher than those of the control group(P < 0.05). While there was no statistically significant difference of the scores of making plans of diagnosis and treatment and the accept attitude between the two groups (P > 0.05). Conclusion In the clinical teaching of obstetrics and gynecology, the application of TSP can improve the clinical practice skills of medical students.

    婦产科学是一门实践性极强的科学,如何切实有效地进行临床实践是妇产科带教老师面临的主要问题。由于妇产科疾病诊治过程中需要行暴露个人私密处的妇科检查,很多患者拒绝成为医学生实习的对象。因此,如何解决医学生在妇产科教学中存在的教学病例少、患者排斥、学生积极性不高、动手操作能力差等问题,是妇产科教学面临的困难。

    标准化病人(standardized patients)是近年来提出的一种新的教学方法。指正常人或者慢性病患者模拟某一疾病的患者,经过训练之后,在学生临床学习或训练过程中能够扮演患者的角色,配合学生的病史采集和体格检查,并且能够表现出某一疾病患者的心理和社会特点。同时,标准化病人根据自己的感受在专门设计的表格上记录并评估学生的操作技能[1]。教师标准化病人(teachers standardized patients,TSP)是由标准化病人而来,指由经验丰富的临床教师代替标准化病人扮演患者[2]。本文就TSP教学与传统教学进行比较,探讨TSP教学在妇产科教学实践中的应用价值。, http://www.100md.com(张媛媛 朱明玥 高冬梅)
