交替床头抬高对神经外科机械通气患者并发症的影响(1)
[摘要] 目的 探究交替床头抬高对神经外科机械通气患者并发症的影响。 方法 选取2018年11月—2019年11月南通大学附属医院神经外科收治的机械通气患者90例,采用随机数字表法分为对照组和观察组,各45例。对照组给予常规床头抬高,观察组给予交替床头抬高,比较两组肺功能改善情況、床头抬高依从性及并发症发生率。结果 干预后,两组动脉血氧分压(PaO2)、氧合指数(OI)均高于干预前,吸入氧浓度(FiO2)低于干预前,且观察组PaO2、OI高于对照组,FiO2低于对照组,差异有统计学意义(P < 0.05);观察组干预7 d后床头抬高依从性高于对照组,差异有统计学意义(P < 0.05);观察组干预7 d后总并发症发生率低于对照组,差异有统计学意义(P < 0.05)。结论 交替床头抬高应用于神经外科机械通气患者中,可改善其肺功能,提高床头抬高依从性,并有效降低并发症发生率,值得推广。

[Abstract] Objective To explore the effect of alternating bed head elevation on the complications of neurosurgical mechanical ventilation patients. Methods Ninety patients with mechanical ventilation admitted to the Department of Neurosurgery, Affiliated Hospital of Nantong University from November 2018 to November 2019 were selected and divided into control group and observation group by random number table method, with 45 cases in each group. The control group was given conventional bed head elevation, and the observation group was given alternating bed head elevation. the improvement of lung function, compliance with altcrnating bed head elevation and incidence of complications in the two groups were compared. Results After intervention, the arterial partial pressure of oxygen (PaO2) and oxygenation index (OI) of both groups were higher than those before intervention, and the inhaled oxygen concentration (FiO2) was lower than that before intervention. Moreover, the PaO2 and OI of the observation group were higher than those of the control group, and FiO2 was lower than that of the control group, with statistically significant differences (P < 0.05); after seven days of intervention in the observation group, the compliance of alternating bed head elevation was higher than that of the control group, and the difference was statistical significance (P < 0.05); the total incidence of complications after 7 days of intervention in the observation group was lower than that in the control group, and the difference was statistically significant (P < 0.05). Conclusion Alternating bed head elevation is used in patients with neurosurgical mechanical ventilation, which can improve lung function, improve bedside elevation compliance, and effectively reduce the incidence of complications. It is worth promoting.
