    摘要 橋本甲状腺炎属于中医学“瘿病”范畴。肝为罢极之本，对气血运行、六经气机、五脏功能起调节作用。瘿病的主要病因是情志不畅，核心致病环节为肝失疏泄，五脏功能及气机升降失常，产生病理产物堵塞于经络。本病的西医治疗目前以甲状腺激素替代疗法为主，但该疗法及其他非特异免疫治疗对于桥本甲状腺炎的免疫损伤进程和伴随的健康危害和生命质量的多系统损害无效。现从中医基础理论出发，结合黄元御“一气周流”理论对从肝论治瘿病的机制进行探讨，提出以疏肝解郁消瘿、缓肝健脾消瘿、暖肝温阳消瘿、柔肝养阴消瘿为治疗法则，并列举相应代表方药柴胡旋覆花汤、柴胡芍药半夏汤、温肝汤、阿胶鸡子黄汤以供参考，以期发挥中医特色、提高临床疗效，对造福桥本甲状腺炎患者有所裨益。
