    [摘要] 目的：促进临床合理用药，保障患者用药安全、有效、经济。方法：借助药物咨询、用药安全监测系统及相关文献对我院静脉用药调配中心配液医嘱的合理性进行审核、分析。结果：常见的不合理医嘱包括溶媒选用不当、剂量浓度问题、给药频次不当、配伍禁忌、中药注射剂使用不当、医嘱录入错误等问题。结论：临床药师可借助静脉用药调配中心平台开展相应的临床药学工作，促进医院临床药学发展。

    Practice and experience of prescription control of clinical pharmacist in pharmacy intravenous admixture services

    [Abstract] Objective: To promote reasonable use of clinical medicine and ensure security, effectiveness and economy of drug use for patients. Methods: Medicine consultation, drug safety monitoring system and related articles were used to examine and verify the rationality of medical order in the pharmacy intravenous admixture service. Results: Solvent improper selection, concentration problems, frequency and joint improper of drug use, inappropriate use of traditional Chinese medicine injection, wrong copy of medical order were included in unreasonable medical order. Conclusion: Clinical pharmacist can work efficiently and promote the development of clinical pharmacy in hospital with the help of pharmacy intravenous admixture services. ......
