原发性高血压患者尿微量白蛋白与左心室损害及血管内皮功能的相关性研究
赵静+魏淑岩+李然+马光+索蒲霞[摘要] 目的 探讨原发性高血压尿微量白蛋白与左心室损害及血管内皮功能的相关性研究。 方法 选择2012年10月~2013年10月来保定市第二中心医院行高血压治疗的门诊和住院的患者60例,作为观察组;选择同期行健康体检者60例,作为对照组。两组入选后1 d内行24 h尿微量白蛋白、内皮依赖性血流介导的血管舒张功能的检测、颈总动脉内膜中层厚度(IMT)超声检查。 结果 观察组反应性充血舒张百分数[(4.8±0.2)%]明显低于对照组[(9.0±1.1)%],差异有统计学意义(P < 0.05)。观察组颈总动脉IMT及硬化斑块指数明显高于于对照组,左心室质量指数低于对照组,差异有统计学意义(t = 9.226,t = 10.005,t = 8.995,P < 0.05)。高血压患者微量白蛋白为58~245 mg/24 h,平均(198.32±33.6)mg/24 h,与肱动脉反应性充血舒张百分数呈负相关(r = -0.6008,P < 0.05),与IMT呈正相关(r = 0.7112,P <0.05)。 结论 原发性高血压患者均存在内皮依赖性血管舒张功能受损、左心室损害,且尿微量白蛋白的发生与内皮依赖性舒张功能和IMT密切相关。

Correlation study between urine trace albumin, left ventricular damage and endothelial function on primary hypertension patients

[Abstract] Objective To study the correlation between urine trace albumin, left ventricular damage and endothelial function on primary hypertension patients. Methods 60 cases of patients with hypertension treatment outpatient and hospitalization in the Second Center Hospital of Baoding City, from October 2012 to October 2013 were chosen as observation group; 60 cases of healthy check-up in the same period were chosen as control group. For two groups, after admission day, 24 h urine trace albumin, endothelial dependent flow mediated vasodilatation function testing, total artery in a medium film thickness (IMT) ultrasound were detected. Results the percentage of diastolic reactive hyperemia in observation group (4.8±0.2)% was obviously lower than that in the control group (9.0±1.1)%, the difference was statistically significant (P < 0.05). IMT and hardening of the plaques in the arteries of the observation group were significantly higher than those of the control group, and LVMI of the observation group was lower than that of control group, the differences were statistically significant (t = 9.226, t = 10.005, t=8.995, P < 0.05). Trace albumin inpatients with high blood pressure were 58-245 mg /24 h, the average of (198.32 ±33.6) mg/24 h, and it was negatively correlated with the percentage of brachial artery diastolic reactive hyperemia (r = -0.6008, P < 0.05), and it was positively correlated with IMT (r = 0.7112, P < 0.05). Conclusion Patients with essential hypertension of endothelium dependent vasodilatation function and left ventricular damage are impaired, and the happening of the urine trace albumin is closely related to the endothelium-dependent relaxing function and IMT. ......
