    [摘要] 目的 比较锥形束CT(CBCT)和牙片两种影像手段评价磨牙根管充填质量，以指导临床医生对影像设备的选择。 方法 选取2012年8月～2013年6月在福建医科大学附属福州市第一医院口腔科2名牙体牙髓专科医师进行根管治疗的患者，经完善治疗后拍摄数字化牙片，对恰填充填物距根尖0～2 mm的患者拍摄CBCT。共收集磨牙50颗，154个根管。数字化牙片和CBCT评价的充填质量以Fisher确切概率法分析。 结果 在数字化牙片上表现为恰填的50颗磨牙在CBCT上显示实际恰填率为70.0%，154个磨牙根管恰填率为87.0%，欠填5.2%，超填7.8%。CBCT与数字化牙片的恰填率比较，差异有高度统计学意义(P < 0.01)。数字化牙片的误诊率达30.0%。 结论 CBCT评价磨牙根充质量比数字化牙片精确，但临床医师应该衡量利弊地使用CBCT。

    Comparative analysis of evaluation for quality of root canal filling of molars by cone bean CT and digital periapical radiography

    [Abstract] Objective To compare the quality of root canal filling of the molars by cone bean computerized tomography (CBCT) and digital periapical radiography (DPR), and to guide the dentists to choose the property imaging devices for root canal treatment (RCT). Methods From August 2012 to June 2013, in Department of Stomatology, the First Hospital of Fuzhou City Affiliated to Fujian Medical University, 50 molars, 154 root canals were randomly selected by 2 endodontic specialists. After property RCT, the quality of the RCT by DPR was evaluated in length. The patients whose adequate filling was 0-2 mm short from the end of the root canal filling to the apex were informed and consent to accept CBCT. The differences of the quality of the RCT between DPR and CBCT were analyzed by Fisher's Exact Test. Results All of the 50 molars were adequate in DPR, but the adequate rate was 70% by CBCT. Counted by root canals, 87.0% were adequate, 7.8% were overextended and 5.2% were underextended of the 154 root canals. The adequate rate of CBCT was compared with DPR, the difference was statistically significant (P < 0.01). The misdiagnosis rate of the DPR with radiographically was 30.0%. Conclusion It is more accurate when the quality of RCT evaluated by CBCT than DPR. However, dentists should weigh the pros and cons to choose the CBCT. ......
