    脑性瘫痪患儿年龄与加强型气管插管内径的相关性研究

    目的探讨脑性瘫痪(以下简称“脑瘫”)患儿年龄与加强型气管插管内径型号的相关性，为该类患儿全身麻醉时选择适宜型号的导管提供理论依据。方法选择2013年6月～2015年12月四川省八一康复中心择期行手术治疗的脑瘫患儿210例，ASA分级Ⅰ～Ⅱ级。按年龄段将患儿分为7组：<1、1～<2、2～<3、3～<4、4～<5、5～<6、6～<7岁，每组各30例。麻醉诱导后，用视频喉镜暴露声门，评估声门的大小并参照公式“ID=2.5+0.4×年龄”选择相应型号的加强型气管插管，并备好相邻的型号。气管插管成功后连接呼吸机行机械通气。记录该患儿年龄与最终选择的导管内径值，分析二者的相关性。结果210例患儿所插加强型气管插管的内径与年龄呈正相关(r=0.949，P<0.01)，回归方程为Y=2.100+0.415X，因此小儿加强型气管插管的内径选择可参考线性回归方程ID=2.1+0.4×年龄。结论在脑瘫患儿的全身麻醉中，患儿年龄与加强型气管插管的内径呈正相关，公式“ID=2.1+0.4×年龄”可能更适合脑瘫患儿加强型气管插管型号的选择。

    [Abstract]Objective To eva1uate the corre1ation of age of chi1dren with cerebra1 pa1sy and interna1 diameter of strengthen endotrachea1 tube,and provide theoretica1 basis for se1ecting appropriate type of strengthen endotrachea1 tube in genera1 anesthesia.Methods From June 2013 to December 2015,210 chi1dren with cerebra1 pa1sy in Sichuan 81 Rehabi1itation Center schedu1ed for e1ective surgery were enro11ed,a11 chi1dren with the American Society of Anesthesio1ogists(ASA)physica1 statusⅠorⅡ.Chi1dren were divided into 7 groups according to age:<1,1-<2,2-<3,3-<4,4-<5,5-<6 and 6-<7 years,with 30 cases in each group.After induction of anesthesia,the g1ottis was exposed with wire1ess video 1aryngoscope,the g1otta1 aperture was eva1uated and the corresponding endotrachea1 tube was se1ected combined with sma11er than traditiona1 mode1s according to the formu1a"ID=2.5+0.4×age".After trachea1 intubation,mechanica1 venti1ation was given after connected to a venti1ator.The va1ues of tube diameter and chi1dren′s age were recorded,and the corre1ation between the two was ana1yzed.Results The corre1ation ana1ysis showed that,chi1-dren′s age was positive1y associated with interna1 diameter of strengthen endotrachea1 tube(r=0.949,P<0.01).The regression equation was Y=2.100+0.415 X,therefore,cou1d be reference.Regression equation and the formu1a of ID =2.1+0.4×age cou1d be used to choose the tube diameter.Conclusion In genera1 anesthesia of chi1dren with cerebra1 pa1sy,the age and interna1 diameter of strengthen endotrachea1 tube show high1y positive corre1ation,the formu1a“ID= 2.1+0.4×age”may be more suitab1e for the se1ection of strengthen endotrachea1 tube in chi1dren with cerebra1 pa1sy. ......
