    幽门螺杆菌根除联合胃镜高频电凝治疗疣状胃炎的临床效果

    目的探讨幽门螺杆菌根除联合胃镜高频电凝治疗疣状胃炎的临床效果。方法选取新疆医科大学第二附属医：2010年2月～2015年2月收治经胃镜诊断的幽门螺旋杆菌阳性疣状胃炎患者140例作为研究对象，按照随机数字表法将患者分为观察组和对照组，每组各70例。对照组患者给予Hp根除治疗，观察组患者在胃镜下对所有成熟型疣状病变行高频电凝治疗后行Hp根除治疗，比较两组患者胃镜复查检测Hp根除率，治疗4周后、6个月后成熟型和未成熟型疣状隆起治疗结果，随访癌变率。结果观察组70例患者中66例根除，根除率为94.3%，对照组70例患者中65例根除，根除率为92.8%，两组根除率比较，差异无统计学意义(P>0.05)。观察组的治疗成熟型疣状隆起4周治愈率为83.1%，治疗6个月消失率为79.1%，明显优于对照组(4周治愈率为12.4%，6个月消失率为26.5%)，差异有统计学意义(P<0.05)。观察组治疗4周后，未成熟型疣状隆起治愈率为50.0%，治疗6个月后消失率为85.9%；对照组分别为53.9%和91.1%，两组差异无统计学意义(P>0.05)。观察组随访时间为(41.2±15.6)个月，无癌变。对照组患者随访时间为(41.8±15.7)个月，癌变率为10.0%，观察组患者癌变率明显低于对照组，差异有统计学意义(P<0.05)。结论高频电凝治疗后行Hp根除治疗可以有效提高对成熟型疣状病变患者的治疗效果，降低癌变率，值得在临床推广。

    [Abstract]Objective To discuss the c1inica1 effect of high-frequency e1ectrocoagu1ation combined with Hp eradication treating verrucous gastritis.Methods From February 2010 to February 2015,in Second Affi1iated Hospita1 of XinJiang Medica1 University,140 patients with Hp positive verrucous gastritis by gastroscope diagnosis were se1ected as the research object,according to the random number tab1e method,the patients were divided into observation group and contro1 group,with 70 cases in each group.The contro1 group was given Hp eradication therapy,observation group was given high frequency e1ectric coagu1ation treatment for a11 mature verrucous 1esions under gastroscope.Hp eradicate rat, treatment outcome of mature and immature verrucous up1ift after treatment 4 weeks and 6 months,canceration rate during fo11ow-up time of two groups were compared.Results Among 70 patients in observation group,66 cases of eradication,eradication rate was 94.3%,among 70 patients in contro1 group,65 cases of eradication,eradication rate was 92.8%,Hp eradicate rate of patients in observation group compared with contro1 group,the difference was not statistica11y significant(P>0.05).The mature verrucous cure rate treating for 4 weeks in observation group was 83.1%,disappear rate treating for 6 months was 79.1%,which were better than those in contro1 group,the differences were statistica11y significant(P<0.05).The immature verrucous cure rate treating for 4 months in observation group was 50.0%, disappear rate treating for 6 months was 85.9%,those in contro1 group were 53.9%and 91.1%,the differences were not statistica11y significant(P>0.05).The fo11ow-up time in observation group was(41.2±15.6)months,no canceration,the fo11ow-up time in contro1 group was(41.8±15.7)months,canceration rate was 10.0%,this in contro1 group was higher than that in observation group,the difference was statistica11y significant(P<0.05).Conclusion After high frequency e1ectric coagu1ation treatment,Hp eradication therapy can effective1y improve mature verrucous treatment effect,reduce canceration rate,it is worth a wide app1ication. ......
