目的观察老年2型糖尿病患者血清25-羟维生素D3水平和胰岛素抵抗的相关性。方法选择2014年1月~ 2015年9月在新疆维吾尔自治区人民医:(以下简称野我:冶)干一科就诊的老年2型糖尿病患者68例作为研究组,随机抽取同期在我:进行体检的健康受试者68例作为对照组。观察两组患者的性别尧空腹血糖尧糖化血红蛋白A1c(HbA1c)尧空腹胰岛素尧血脂尧血清25-羟维生素D3尧胰岛素抵抗指数等,分析血清25-羟维生素D3水平与临床资料的相关性。结果两组性别尧年龄尧体重指数(BMI)尧舒张压比较,差异无统计学意义(P跃0.05)曰收缩压尧总胆固醇(TC)尧三酰甘油(TG)尧低密度脂蛋白胆固醇(LDL-C)尧高密度脂蛋白胆固醇(HDL-C)尧空腹血糖尧HbA1c尧25-羟维生素D3尧空腹胰岛素尧胰岛素抵抗指数比较,差异均有高度统计学意义(P约0.01)。血清25-羟维生素D3水平与性别尧年龄尧BMI尧收缩压尧舒张压尧TC尧TG尧LDL-C尧HDL-C等变量均无相关性(P跃0.05),与空腹血糖尧HbA1c尧空腹胰岛素和胰岛素抵抗指数呈明显负相关(r=-0.354尧-0.378尧-0.390尧-0.318,均P约0.05)。结论血清25-羟维生素D3水平降低与糖尿病的发病有关,可能与血清25-羟维生素D3水平降低导致胰岛素抵抗升高有关,提示提高血清25-羟维生素D3水平可作为治疗老年2型糖尿病的临床辅助方法。

[Avsteact]Ovjective To investigate the relationship between serum level of 25-hydroxy vitamin D3and insulin resis—tance in elderly patients with type 2 diabetesmellitus.M ethods Sixty-eight patients in the People's Hospital of Xin—jiang Uygur Autonomous Region(野our hospital冶for short)from January 2014 to September 2015 were selected as study group,68 healthy subjects in our hospitalwere selected as control group at the same period.The general,fasting blood glucose,glycosylated hemoglobin(HbA1c),fasting insulin,blood lipid,serum 25 hydroxyvitamin D3,insulin resistance index of two groupswere observed;the correlation of serum 25 hydroxyvitamin D3level and clinical data was analyzed. Results The gender,age,bodymass index(BMI),diastolic pressure of two groupswere compared between two groups, therewas no significant difference(P>0.05).The systolic blood pressure,TC,TG,LDL-C,HDL-C,fasting blood glu—cose,HbA1c,25 hydroxy vitamin D3,fasting insulin,insulin resistance index in the two groups had statistically signifi—cant differences(P0.05);serum level of 25 hydroxyvitamin D3was significantly negatively related with fasting blood glucose,HbA1c,fasting insulin and insulin resistance index(r= -0.354,-0.378,-0.390,-0.318,all P1 资料与方法 ......
