    [摘要] 目的 研究鼻內镜检查在儿童鼻窦炎诊断中的应用价值。 方法 选取佛山市妇幼保健院耳鼻咽喉科2015年6月～2016年5月收治的90例鼻窦炎患儿作为研究对象，在征得患儿家属知情同意下先后进行鼻内镜、鼻窦CT或磁共振检查，收集鼻内镜Lund-Kennedy评分以及影像学Lund-MacKay评分，采用Spearman秩相关对Lund-Kennedy评分与Lund-Mackay评分进行相关性分析，应用Logistic回归对鼻内镜下各种体征进行多元回归分析。 结果 经Spearman检测，Lund-Kennedy评分与Lund-Mackay评分的Spearman相关系数r=0.91，P<0.01，两者高度相关；Logistic回归提示，上、中鼻道引流为鼻内镜检查中诊断鼻窦炎的影响因素(OR=5.825，95%CI：2.199～15.431，P < 0.01)；症状+鼻内镜诊断敏感度(57.63%)、特异度(84.85%)、阳性预测值(91.89%)、阴性预测值(52.83%)，与单纯症状诊断敏感度(76.27%)、特异度(32.25%)、阳性预测值(68.18%)、阴性预测值(41.67%)比较，差异有统计学意义(P < 0.05)。 结论 鼻内镜检查能够在很大程度上替代鼻窦影像检查且检测费用低，可作为临床诊断的优选方案推广应用。
