    [摘要] 探索临床药师参与危重症患者抢救的药学实践切入点，通过参与危重症患者诊疗全过程，结合肾功能情况，关注药物相互作用，及时干预调整药物治疗方案，使临床药师在药物治疗上发挥自身专业优势，协助医生优化治疗方案，在临床治疗团队中发挥重要作用，实现住院患者药物治疗全程个体化。

    [Abstract] To explore the breakthrough point of pharmacy practice of clinical pharmacists participating in the rescue of critically ill patients， by participating in the diagnosis and treatment process of critically ill patients， combining the conditions of kidney function， paying attention to drug interactions， timely adjusting the drug treatment interventions， the clinical pharmacists in drug therapy play their own professional advantages， assist doctors to optimize the treatment plan， play an important role in the clinical treatment team， realize the hospitalized patients through individualized drug treatment.
