    [摘要] 目的 探討颈前路治疗下颈椎骨折脱位并脊髓损伤的临床效果。 方法 选取2010年1月～2017年3月宝鸡市中心医院收治30例下颈椎骨折脱位并脊髓损伤的患者，行颈前路减压复位、植骨融合、内固定术。评估术后颈椎生理曲度、植骨融合程度及神经功能恢复情况。 结果 所有患者随访12～24个月，平均(13.5±1.5)个月，植骨均在术后6个月获得骨性融合，颈椎椎间高度及生理曲度维持良好，内固定物未出现断松动、断裂。术后6个月颈椎Cobb角测量比术前明显改善，差异有高度统计学意义(P = 0.000)。根据Frankel分级，B～D级患者较术前有恢复，A、E级患者变化不明显。术后与术前比较，JOA评分升高，差异有高度统计学意义(P = 0.000)。 结论 下颈椎骨折脱位并脊髓损伤应用前路减压植骨融合内固定可重建颈椎稳定性，恢复颈椎的高度及生理曲度，解除脊髓压迫，促进神经功能恢复。
