    [摘要] 心脏康复作为冠心病治疗临床路径中的重要组成部分，目前不能满足患者的康复需求。基于医院的延续护理是研究最多、发展最好的延续护理模式，可促进冠心病患者参与心脏康复过程，改善心脏康复成效，有益于提升患者生活质量，降低心血管不良事件发生。本文就心脏康复和延伸护理的概念、基于医院的延伸护理应用于心脏康复的背景和应用现状进行综述，以期为冠心病心脏康复的临床实践研究提供参考。
