【摘要】 目的 分析老年人股骨粗隆间骨折的治疗情况，评价不同治疗方法的疗效。方法 47例老年人股骨粗隆间骨折保守治疗6例。手术治疗41例，其中36例为DHS内固定治疗。结果 术后随访6～27个月，平均12.4个月。无感染及骨折不愈合发生，1例DHS内固定的患者出现螺纹钉穿出股骨头，2例出现髋内翻。髋关功能根据改良Harris式评分方法进行疗效评价。本组优：36例，良：7例，可：3例，差：1例。优良率91.49%。结论 手术治疗是老年人股骨粗隆间骨折的首选方法；DHS是较为理想的内固定方法。

    Freatment of treatment of intertrochanteric fracture of the femur in the elder

    【Abstract】 Objective To explore retrospectively the treatment effects of intertrochanteric fractures of the femur in the elder using dif erent methods.Methods Forty seven cases with intertrochanteric fracture had been treated with different methods in the elder between June 2000 and January 2006.6 out of 47 cases were treated with conservative therapy the other.41 cases were operated on.36 out of 41 were performed with DHS internal fixation(Dynamic Hip Screw).Results There was not any infection and nonunion cases.Screw perforated the femoral head in 1 case of the DHS internal fixation.Coxa vara was found in 2 cases.The mean follow-up period was 12.4 months,ranging from 6 to 27 months.The outcomes of the treatment were graded the excellent,the good,the fair and the poor according to the standard of joint function of hip basing on the modified Harris scoring system.The excellent was 36,the good 7,the fair 3 and the poor 1.The rate of both the excellent and the good of the outcomes was 91.49%(43/47)out of the 47 cases.Conclusion Operation is the first selection for treating intertrochanteric fractures of the femur in elderly and the DHS is an ideal internal fixation method. ......
