乳腺癌患者血清胰岛素样生长因子-1术前及术后水平检测意义研究

【摘要】 目的 检测乳腺癌患者术前及术后血清胰岛素样生长因子-1的水平。方法 应用双位点免疫放射测定法(IRMA)对不同TNM分期的49例乳腺癌患者术前、术后外周血中IGF-1水平进行检测，20例健康志愿者设为正常对照。结果 乳腺癌患者手术前后血清IGF-1水平均高于正常对照(P<0.05)，手术前后乳腺癌患者血清IGF-1水平差异无统计学意义(P>0.05)。结论 乳腺癌患者血清IGF-1参与乳腺癌的发病过程，是乳腺癌发生、发展过程中的重要因子。

【Abstract】 Objective
To detect the serum insulin-like growth factor-1(IGF-1)levels in patients with breast cancer before and after operation and explore the feasibility of its clinical significance. Methods Immunoradiometric assay(IRMA)was used to detect the IGF-1 level in peripheral blood in 49 pre-and post-operative patients with breast cancer(based on TNM staging).20 healthy subjects were taken as control group. Results The serum IGF-1 levels of pre-and post-operative patients with breast cancer were higher than that in control group(P<0.05).Serum IGF-1 level in the patients with breast cancer had no significantly difference before operation(P>0.05). Conclusion IGF-1 may be involved in the oncosis and development of breast cancer.

乳腺癌是女性常见的恶性肿瘤，虽然近年来治疗手段有了较大的进步但仍是导致女性死亡的主要原因，研究表明多种生物因子参与乳腺癌的发生、发展及转移，其中胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)在肿瘤的发生发展及预后评价中起着重要的作用。研究表明大肠癌、前列腺癌、乳腺癌等癌症患者外周血IGF-1水平明显升高[1,2]。本研究应用双位点免疫放射测定法(IRMA)对不同TNM分期的49例乳腺癌患者术前、术后外周血中IGF-1进行检测，以探讨IGF-1在乳腺癌发生、发展过程中的作用。
1 资料与方法
1.1 一般资料
1.1.1 标本 所有49例标本均来自本院2007年5月至2008年9月住院拟手术的患者，入院前均无放化疗治疗史，并取得患者同意签署血液样本使用同意书。其中Ⅰ期12例，Ⅱ期30例，Ⅲ期7例，患者于入院后及术后7 d各抽取静脉血5 ml，并设健康志愿者对照20例。
1.1.2 主要试剂 所需试剂：IGF标准液，125I标记的抗IGF-1，抗IGF-1包被的反应管，IGF-1质控液及稀释缓冲液和含非离子清洁剂的清洗液购自Invitrogen公司。
1.2 方法 IGF-1的测定，将1 ml稀释液加入质控管及标准管中，将56 μl标准液及质控物和1∶40稀释的待测血清加入含抗IGF-1包被的反应管中混匀，随即向管中加入125I标记的抗IGF-1液200 μl，轻柔晃动，混匀，并将反应管放震荡器上在室温下(15℃~28℃，356 r/min)作用2 h，除总放射性测定管外，将其余各管离心，弃去上清液用小枪头或吸水纸吸干管内残留液体，加清洗液2 ml，洗去残留未结合的125I标记的抗IGF-1，共洗涤3次，加入SN-6105γ放射免疫计数仪计数，IGF-1的含量通过标准曲线求出。
1.3 统计学方法
使用SPSS12.0软件进行统计学处理，与正常对照组之间的比较采用独立样本的t检验 ......
