TCD下血流动力学与血管性认知功能障碍患者认知功能的相关性研究(1)
【摘要】 目的 探讨经颅多普勒(TCD)下血流动力学与血管性认知功能障碍患者认知功能的关系。方法 40例血管性认知功能障碍患者作为观察组, 另取40例认知功能正常的血管性疾病患者作为对照组。两组患者均实施TCD检查, 分析两组基线资料以及血流动力学相关指标。结果 两组患者年龄、男女分布、受教育程度方面比较差异无统计学意义( P>0.05)。观察组大脑中动脉搏动指数为(1.12±0.17), 大脑前动脉搏动指数为(1.03±0.24), 明显高于对照组的大脑中动脉搏动指数(0.81±0.20)、大脑前动脉搏动指数(0.77±0.19), 差异有统计学意义(P<0.05) ;观察组大脑中动脉血流速度、大脑前动脉血流速度分别为(48.21±6.18)、(37.10±5.65)cm/s, 与对照组大脑中动脉血流速度、大脑前动脉血流速度(46.07±6.52)、(36.82±4.89)cm/s比较, 差异无统计学意义(P>0.05)。结论 采取TCD检查, 血管性认知功能障碍患者的大脑动脉搏动指数明显更高, 与其认知功能障碍程度具有一定相关性。

【Abstract】 Objective To investigate relationship between transcranial color Doppler (TCD)-guided hemodynamics and cognitive function in vascular cognitive impairment patients. Methods There were 40 patients with vascular cognitive impairment as observation group and 40 patients with vascular disease and normal cognitive function as control group. Both groups received TCD examination for analysis of their baseline information and hemodynamics related indexes. Results There was no statistically significant difference of age, gender distribution and education level between the two groups (P>0.05). The observation group had middle cerebral artery pulsatility index as (1.12±0.17) and anterior cerebral artery pulsatility index as (1.03±0.24), which were all much higher than middle cerebral artery pulsatility index as (0.81±0.20) and anterior cerebral artery pulsatility index as (0.77±0.19) in the control group, and their difference had statistical significance (P<0.05). The observation group had middle cerebral artery flow velocity and anterior cerebral artery flow velocity respectively as (48.21±6.18) and (37.10±5.65) cm/s. Comparing with ity and anterior cerebral artery flow velocity in the control group as (46.07±6.52) and (36.82±4.89) cm/s, their difference had no statistical significance (P>0.05). Conclusion Implement of TCD examination shows remarkably higher cerebral artery pulsatility index in patients with vascular cognitive impairment, which contains certain correlation with cognition impairment degree.
