【摘要】 目的 探讨颅内破裂动脉瘤早期手术治疗总结临床经验。方法 回顾我科2006年7月至2009年7月经早期手术治疗28例颅内破裂动脉瘤的临床资料,均在24 h内行动脉瘤夹闭术,全部病例均采用Yasargil入路,手术分离和夹闭动脉均在显微镜下进行,同时采用临时阻断载瘤动脉的方法预防和处理术中发生的动脉瘤破裂。结果 所有动脉瘤均给予夹闭,其中瘤颈未闭23例,瘤颈夹闭+包裹3例,单纯包裹2例,按GOS评分评价预后,恢复良好26例(92.9%),中残2例,无重残及死亡病例。结论 颅内破裂动脉瘤早期经显微手术治疗效果好。

    Clinical Effect Analysis of Microsurgical treatment for the anterior circulation aneurysms through pterion approach

    【Abstract】 Objective To investigate the early surgical treatment of ruptured intracranial aneurysms.Methods To retrospect 28 cases of ruptured intracranial aneurysms with early surgical treatment from July 2006 to July 2009.All aneurysms were incarcerated within 24 hours through Yasargil approach.Microscope was used in the operations.Blocking parent arteries temporarily were used to prevent rupture of aneurysms.Results All aneurysms were clamped successfully.Aneurysm neck in 23 cases were patent,3 cases were clamped and encapsulated,2 cases were simply encapsulated.According to GOS,26 cases got good recovery,2 moderately disabled,and no severely disabled or death.Conclusion The early surgical treatment of ruptured intracranial aneurysms has good effect. ......
