心脏超声评价心室收缩同步性的临床效果研究(1)

[摘要] 目的 探讨心脏超声评价心脏收缩同步性的临床效果。方法 选择2007年2月～2010年3月孤立性左束支传导阻滞患者36例为观察组，40例心电无异常的健康人为对照组，对所有试验者的心率(HR)及心脏超声指标LVEF、LVEDV、Tmsv16-SD、Tmsv16-Dif、Tmsv6-SD及Tmsv6-Dif进行单因素对比分析，然后对与心脏收缩同步性的相关因素进行Logistic多元回归分析。结果 单因素分析提示观察组与对照组在HR、LVEF、LVEDV无明显差异(P>0.05)；观察组与对照组在Tmsv16-SD、Tmsv16-Dif、Tmsv6-SD及Tmsv6-Dif存在明显差异(P<0.05)。Logistic多因素回归分析显示Tmsv16-SD、Tmsv16-Dif、Tmsv6-SD及Tmsv6-Dif是评价心室收缩同步性的独立因素。结论 超声心动能较准确地评价心室收缩的同步性，尽管左束支传导阻滞者的射血分数尚正常，但以超声心动为标准的心功能已有下降。

The Clinical Research of Heart Ultrasound in Cardiac Synchronicity

[Abstract] Objective To evaluate the value of heart ultrasound to predict cardiac synchronicity. Methods 36 patients with left bundle branch block were chosen from February 2007 to March 2010 as the test group and 40 healthy people as the control group. The heart ultrasound data of all patients were analyzed with multiple logistic regression analysis. Results Compared with the control group，HR，LVEF，LVEDV in test group had no statistically significant difference(P>0.05). Compared with the control group，Tmsv16-SD，Tmsv16-Dif，Tmsv6-SD and Tmsv6-Dif in test group had statistically significant difference(P<0.05). Tmsv16-SD，Tmsv16-Dif，Tmsv6-SD and Tmsv6-Dif are independent factors for predicting cardiac synchronicity. Conclusion Heart ultrasound can evaluate the synchronization of the myocardium accurately.

超声心动是应用最为广泛的评价心脏运动同步性的无创性手段，不但具有可靠、可重复的优点，而且能够定量不同心肌节段的运动时间和速度，提供电机械偶联的准确信息[1]。以往国内的研究人群主要是严重心功能不全患者，较少以孤立性束支传导阻滞者为研究对象。孤立性左束支传导阻滞都存在显著的心室不同步收缩，并且随着QRS时限的延长而增大[2]。本文以孤立性左束支传导阻滞者为研究对象，探讨心脏超声在评价心室收缩同步性的临床效果，现报道如下。
1资料与方法
1.1一般资料
选择本院2007年2月～2010年3月体检时发现的孤立性左束支传导阻滞者36例，其中男21例，女15例，年龄45～73岁，平均年龄(62.5±3.5)岁。患者入组标准：①QRS时限>120ms；②V1、V2导联呈QS型或仅有极小的R波 ......
