糖尿病肾病患者血清脂联素与尿转铁蛋白的检测(1)

     [摘要] 目的 探讨糖尿病肾病患者血清脂联素及尿转铁蛋白检测的意义。方法 选取2009年1月~2011年1月入住我院的2型糖尿病患者75例,根据有无肾功能受损及肾功能受损程度分为正常蛋白尿组、微量蛋白尿组、大量蛋白尿组,另选择25例健康体检者作为对照组。观察所有研究对象的尿微量白蛋白(mAlb)、尿转铁蛋白(TRF)、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、甘油三酯(TG)、血清脂联素(APN)。结果 四组mAlb/Cr比较差异有统计学意义(F=9.84,P<0.05),四组TRF/Cr比较差异有统计学意义(F=10.76,P<0.05),正常蛋白尿组TRF/Cr较对照组明显升高(P<0.05),微量蛋白尿组、大量蛋白尿组mAlb/Cr、TRF/Cr明显高于正常蛋白尿组(P<0.01);四组APN比较差异有统计学意义(F=9.26,P<0.05),正常蛋白尿组APN较对照组明显降低(P<0.05),微量蛋白尿组、大量蛋白尿组APN明显高于正常蛋白尿组(P<0.05)。结论 尿转铁蛋白能更敏感地反映糖尿病患者肾小球的损伤,可作为诊断糖尿病肾病早期损伤的灵敏指标;而血清脂联素可能是反映糖尿病肾病患者微血管病变的生物标记物,血清脂联素水平的升高可能提示糖尿病肾病的发生或加重。

    The Detection of Serum Adiponectin and Urine Transferrin of Diabetes Nephrosis Patient

    [Abstract] Objective To explore the significance of the detection of serum adiponectin and urine transferrin of diabetes nephrosis patient. Methods We selected 70 cases of diabetes nephrosis patients who were in our hospital during January 2009 to January 2011. According to whether kidney damage and the degreeof kidney damage,the patients were divided into normoalbuminuria group, trace proteinuria group,a large number of proteinuria group. 25 healthy persons were selected as control group. To observe the mAlb,TRF,FBG,HbA1c,TG and APN of all subjects. Results The mAlb/Cr of four groups was statistically different(F=9.84,P<0.05),and the TRF/Cr of four groups was statistically different(F=10.76,P<0.05). TRF/Cr of normoalbuminuria group was significant increased than control group(P<0.05),TRF/Cr of trace proteinuria group and a large number of proteinuria group was significant increased than normoalbuminuria group(P<0.01).
