联合应用不同根管充填糊剂治疗224例慢性根尖周炎临床观察(1)

[摘要] 目的 观察联合应用不同根管充填糊剂对于慢性根尖周炎的疗效。方法 随机用不同根管充填糊剂加牙胶尖作为根充材料根管治疗224例慢性根尖周炎患者。结果 Cortisomol糊剂联合Vitapex糊剂的根充剂组术后一周疼痛发生率(3.57%)及1年随访失败率(0%)均低于其他三组(P<0.05)。结论 在慢性根尖周炎的治疗方法中，Cortisomol糊剂和Vitapex糊剂联合使用要优于单用，而氧化锌丁香油糊剂的疗效最低。在临床上推荐使用Cortisomol糊剂联合Vitapex糊剂做为根充材料根管治疗慢性根尖周炎。

Different Root Canal Sealers in the Treatment of Chronic Periapical Periodontits：A Clinical Observation of 224 Cases

[Abstract] Objective To observe the effect of the combined application of different root canal filling paste for the treatment of chronic periapical periodontitis. Methods Different root canal filling paste plus gutta-percha point as a root canal treatment of root canal filling material 224 cases of chronic apical periodontitis patients. Results Cortisomol joint Vitapex paste paste root filling agent group the incidence of pain after a week(3.57%) and one year follow-up failure rate(0%) were lower than the other three groups(P<0.05). Conclusion In chronic periapical treatment，it is better than single Cortisomol paste or Vitapex paste，and the efficacy of zinc oxide eugenol paste is the lowest. Recommended in the clinical joint Vitapex paste and Cortisomol paste as root canal filling materials，root canal treatment of chronic periapical inflammation.

慢性根尖周炎是指根管内由于长期有感染及病原刺激存在，根尖周围组织包括牙骨质、牙周膜和牙槽骨等呈现慢性炎症反应，表现为炎性肉芽组织形成和牙槽骨破坏[1]。在机体抵抗力下降时，可转化为急性根尖周炎或称慢性根尖周炎急性发作，因此，慢性根尖周炎常有反复疼痛、肿胀病史。目前其治疗以根管治疗配合根管充填为主。根管充填材料有牙胶尖、银尖及塑料尖等，常配合糊剂使用。而不同糊剂之间的疗效又有不同。本文拟对比4种不同糊剂作为根充剂根管治疗224例慢性根尖周炎的疗效，现将有关资料报道如下。
1资料与方法
1.1一般资料
收集我院2007年12月～2009年12月收治的224例慢性根尖周炎采用根管治疗的患者作为观察对象，其中男142例，女82例，年龄最小26岁，最大77岁，平均(43.00±12.31)岁。均行牙片检查发现有根尖骨质区破坏较大，需行根管治疗。224例患者共有患牙273颗。随机将患者分为四组：A组56例，采用Cortisomol糊剂联合Vitapex糊剂；B组56例采用Cortisomol糊剂；C组56例采用Vitapex糊剂；D组56例采用氧化锌丁香油糊剂。四组患者的一般资料均无统计学差异，具有可比性。
