舒芬太尼联合帕瑞昔布钠用于胸外科术后患者自控静脉镇痛的临床应用研究(1)

[摘要] 目的 探讨舒芬太尼联合帕瑞昔布钠用于胸外科术后患者自控静脉镇痛(PCIA)的有效性及安全性。 方法 将70例胸外科手术患者随机分为两组，实验组(联合组)和对照组(单纯舒芬太尼组)各35例。手术结束后两组分别使用舒芬太尼自控静脉镇痛，实验组术后静脉注射40mg帕瑞昔布钠，12h后再注射40mg帕瑞昔布钠；对照组术后静脉注射10mL生理盐水，12h后再注射10mL生理盐水。分别于术后6、24、48h观察病人VAS动态评分、舒芬太尼用量、心率、呼吸及血氧饱和度变化情况。记录24h后PCA的次数、药物不良反应发生率及患者对镇痛效果的满意度。 结果 两组患者药物不良反应及生命体征变化无显著差异(P >0.05)。与对照组比较，实验组VAS动态评分、舒芬太尼用量及PCA次数明显下降，患者对镇痛效果的满意度提高(P <0.05)。结论 舒芬太尼联合帕瑞昔布钠用于胸外科术后PCIA效果确切，优于单纯舒芬太尼组，能够在临床上推广使用。

Sufentanil Combined with Parecoxib Sodium for the Treatment of Postoperative Patient Controlled Thoracic Surgery Intravenous Analgesic QIAN Yufang

[Abstract] Objective To study the efficacy and safety of the sufentanil combined with parecoxib sodium in the treatment for thoracic surgery patient-controlled intravenous analgesia (PCIA) after surgery. Methods Seventy cases of thoracic surgery were randomly divided into two groups, the experimental group (combination group) and the control group(simple sufentanil group) with 35 cases in each. After surgery, two groups used sufentanil for controlled intravenous analgesia, the experimental group were then injected with 40mg parecoxib sodium, and used again after 12h; the control group intravenous injected 10mL saline, and repeated once time after 12h. The dynamic of VAS score, sufentanil consumption, heart rate, breathing and pulse oxygen saturation changes were observed respectively after 6h, 24h, 48h. 24h and the number of PCA, the incidence of adverse drug reactions and patient satisfaction with analgesia were recorded. Results The changes of vital signs and adverse drug reactions had no significant difference(P>0.05).
