[摘要] 目的 为提高重型颅脑损伤患者的临床治愈率，本研究旨在探讨应用中西医结合疗法治疗重型颅脑损伤的疗效，并进一步探讨中西医结合疗法对重型颅脑损伤患者免疫功能的机制的影响。 方法 入选确诊的64例重型颅脑损伤患者，随机分为研究组30例(中西医治疗)和对照组34例(西医治疗)。 结果 研究组的总有效率明显高于对照组，差异有统计学意义(χ2=8.231，P<0.05)，研究组的免疫功能各项指标(IFN-r、IL-4、IFN-r/IL-4)在治疗的第1，3，7，14天均较对照组明显改善，差异有统计学意义。 结论 中西医结合疗法对重型颅脑损伤患者疗效确切，且能明显改善患者的免疫功能，值得推广和应用。

Influence of combined therapy of Chinese and Western medicine for immune mechanism on patients with severe traumatic brain injury

[Abstract] Objective To observe the clinical effect of combined therapy of Chinese and Western medicine in treatment of severe brain injury patients, and further to explore the effect of this method on immune function of severe brain injury patients. Methods All of 64 cases selected for diagnosis in patients with severe traumatic brain injury were randomly divided into study group of 30 patients (combined therapy of Chinese and Western medicine) and the control group of 34 patients (western medicine). Results The total efficiency rate of the study group was significantly higher than the control group, and the difference was statistically significant(χ2=8.231, P<0.05). The immune function indicators (IFN-r, IL-4, IFN-r/IL-4) of the research group treatment the first day 1, 3, 7, 14 was significantly improved compared with the control group, and the difference was statistically significant. Conclusion Integrative therapy is effective in patients with severe traumatic brain injury, and can significantly improve the patient's immune function, should be promoted and applied.

重型颅脑损伤是神经外科的常见疾病之一，近年来有文献报道[1]中西医结合治疗有利于促进重型颅脑损伤患者昏迷的苏醒、神经功能的恢复和减少并发症。而重型颅脑损伤后因其全身处于应激状态，长期昏迷，免疫功能受抑制，极易造成感染及脓毒血症，使病情急剧恶化，是重型颅脑损伤患者致死的重要原因之一。因此，重型颅脑损伤患者免疫功能的变化对其预后的影响至关重要，免疫功能下降是导致感染、加重病情的主要原因之一，其中在特异性细胞免疫功能改变中，主要表现为T细胞亚群数量及杀菌能力的降低[2]。因此，为提高临床治愈率，降低术后并发症的发生率，本研究旨在探讨应用中西医结合疗法治疗重型颅脑损伤的疗效 ......
