呼吸训练治疗不同病程稳定期COPD患者的研究(1)

     [摘要] 目的 探讨呼吸训练治疗不同病程稳定期慢性阻塞性肺疾病(COPD)患者的疗效。 方法 根据COPD确诊时间,分为病程<10年和病程≥10年两组。评估各组治疗前和治疗后6个月的肺功能变化[第1秒最大呼气量(FEV1)、FEV1占预计值百分比(FEV1%)和FEV1/用力肺活量(FVC)]、呼吸困难分级(MRC)和运动耐力[6min步行距离(6MWD)]。 结果 病程≥10年组治疗前FEV1、FEV1%、FEV1/FVC和6MWD显著低于病程<10年组,MRC评分显著高于病程<10年组,差异均有统计学意义(P<0.05);病程<10年组 FEV1、FEV1%、FEV1/FVC和6MWD治疗后6个月较治疗前明显升高,MRC较治疗前明显降低,差异均有统计学意义(P<0.05)。 结论 呼吸训练可以改善病程<10年患者的上述指标,但对于病程≥10年患者上述指标无明显改善。

    Effect of respiratory rehabilitation training in the stable chronic obstructive pulmonary disease patients with different disease course

    [Abstract] Objective To evaluate the efficacy of respiratory rehabilitation training in the stable chronic obstructive pulmonary disease (COPD) patients with different disease course. Methods All of 157 patients with stable COPD were divided into two groups according to disease course. If the disease course was ≥ 10 years,was named the observed group,if not was named the compared group. The FEV1,FEV1%,FEV1/FVC,the degree of dyspnea evaluated by medical research council (MRC) dyspnea scale and exercise tolerance evaluated by six minute walking distance (6MWD) were measured before therapy, posttreatment sixth months. Results Before therapy,the FEV1,FEV1%,FEV1/FVC,MRC and 6MWD were in worse condition in the observed group than that the compared group. Posttreatment sixth months, the FEV1,FEV1%,FEV1/FVC,MRC and 6MWD in the observation group did not change significantly,but the compared group improved significantly. Conclusion Respiratory rehabilitation training could improve respiratory function,the degree of dyspnea and exercise tolerance in COPD patients whose disease course < 10 years, but that of the patients whose disease course ≥ 10 years could not improve.
