OSCE考核在住院医师规培中的作用探讨(1)
[摘要] 目的 探讨客观结构化临床考核(objective structural clinical examination，OSCE)在住院医师规培中的作用。方法 采用回顾性分析的方法，将2015年10月～2016年10月在我院接受规范化培训的56例住院医师作为研究对象，所有住院医师均给予OSCE考核，对培训前后不同站点住院医师的OSCE得分、住院医师的认同情况以及住院医师的满意度进行综合评价。 结果 在规范化培训中应用OSCE考核后，发现住院医师在病史采集、病历书写、基本操作技能等站点的OSCE得分显著优于OSCE考核应用前，差异有统计学意义(P<0.05)。OSCE考核后，住院医师对OSCE考核的认可度显著优于应用前，差异有统计学意义(P<0.05)。通过对OSCE考核应用前后住院医师满意度进行分析，结果显示应用后满意度显著优于应用前，差异有统计学意义(P<0.05)。 结论 OSCE考核在住院医师规范化培训中的应用，能够有效提升培训效果，增强住院医师的临床技能，提升住院医师的综合素质，值得广泛推广应用。

[Abstract] Objective To explore the role of objective structural clinical examination(OSCE) in resident physician standardized training. Methods A total of 56 resident physicians who received standardized training in our hospital from October 2015 to October 2016 were enrolled by retrospective analysis. All residents were given OSCE assessment. The OSCE scores of the resident physicians at different sites before and after the training， the resident physician's identity and the satisfaction of the resident were evaluated synthetically. Results The OSCE scores of the resident physician in the history acquisition， medical record writing， basic operating skills， etc. after the application of OSCE assessment in standardized training were significantly better than the those before the application of OSCE assessment(P<0.05). The degree of accreditation of the resident physician to the OSCE after the OSCE assessment was significantly better than that before the application， with statistically significant difference (P<0.05). The satisfaction degree after the application was significantly better than that before the application through the analysis of the satisfaction degree of the resident before and after OSCE assessment， which was statistically significant(P<0.05). Conclusion The application of OSCE in standardized training of resident physicians can effectively improve the training effect， enhance the clinical skills and the comprehensive quality of the residents. It is worthy of popularization and application.

作為临床中一种评估医师临床技能的方法，OSCE在多个国家及地区均被广泛应用，其强调对住院医师临床操作技能的培养，要求考核方式能够满足社会发展及人才培训的需求，是培养高素质临床医师的重要途径[1-3]。近年来，我院在住院医师规范化培训中引进了OSCE考核模式，为探讨其临床作用，研究选取我院56例接受规范化培训的住院医师进行综合分析，并对研究结果进行相应总结。
1 资料与方法
1.1一般资料
研究选取2015年10月～2016年10月在我院接受规范化培训的56例住院医师作为研究对象，自2016年2月应用OSCE考核，其中男31例，女25例，年龄23～30岁，平均(26.32±1.63)岁，其中内科21例(37.5%)，外科20例(35.7%)，儿科15例(26.8%)。56例住院医师中研究生20例(35.7%)，本科生36例(64.3%)。此次研究经过医学伦理会的支持，所有住院医师均自愿参与本次研究，签署知情同意书。, 百拇医药(徐立新 胡文娟 熊丽芳)
