改进创新的诊间预约挂号模式对我院门诊管理的影响(1)
[摘要] 目的 研究经改进创新的诊间预约挂号模式对我院门诊管理的影响。 方法 总结开展改进创新的诊间预约前后(2017年6～9月与2018年6～9月)我院门诊总预约率及患者就诊满意度的数据并进行统计学分析。 结果 ①诊间预约开展后门诊总预约率明显提高，差异有显著统计学意义(P=0.000)。②诊间预约开展后门诊患者就诊满意度明显提高，差异有显著统计学意义(P=0.000)。③诊间预约开展后门诊患者候诊时间缩短，差异有统计学意义(P=0.021)。 结论 改进创新的诊间预约挂号模式提升了医院的整体服务水平，是医改后提高门诊服务质量的有益探索和实践。

[Abstract] Objective To study the effect of improved innovative inter-visit appointment and registration mode on the outpatient management of our hospital. Methods The data of the general appointment rate and patient’s satisfaction in our hospital before and after the implementation of the improved innovative inter-visit appointment(June to September 2017 and June to September 2018) were summarized and statistically analyzed. Results (1)The total appointment rate of the outpatient clinic was significantly improved after the implementation of inter-visit appointment， and the difference was statistically significant(P=0.000). (2)After implementation of the inter-visit appointment， the satisfaction of the outpatients was significantly improved， and the difference was statistically significant(P=0.000). (3)After the implementation of the inter-visit appointment， the waiting time of outpatients was shortened， and the difference was statistically significant(P=0.021). Conclusion The improved innovative inter-visit appointment and registration mode has improved the overall service level of the hospital， which is a useful exploration and practice to improve the quality of outpatient service after medical reform.

北京市大兴区人民医院是一所县域级三级医院，是北京市大兴区的龙头医疗机构。其直接服务于大兴区群众，并辐射周边地区，近几年来全年门诊量均在160万人次以上。2017年4月北京市推动医改后，作为县域级医疗中心，如何开拓创新改善群众就医感受、解决“看病难”成为我院首要课题。门诊是医院对外公开的部门，反映了一家医院的整体水平，门诊管理优劣对于改善群众就医感受、解决“看病难”非常重要[1]。而不断完善预约挂号模式，使患者方便、快捷、正确地进行挂号就是门诊管理水平的重要体现。改进创新的诊间预约挂号模式就是我院医改后门诊优化流程、 提高门诊服务质量的有益探索和实践。我院从2017年10月起实行了诊间预约挂号方式，现报道如下。
1 资料与方法
1.1 一般资料
选择2017年6～9月我院门诊总量534559例次，设定为诊间预约未实施A组;2018年6～9月我院门诊总量537018例次，设定为诊间预约实施A组。以做两组门诊总预约率比较。
另随机选取2017年～9月我院门诊治疗的1000例患者作为诊间预约未实施B组，其中男562例，女438例，年龄26～78岁，平均(56.9±13.4)岁;选取2018年6～9月门诊治疗的1000例患者作为诊间预约实施B组，其中男531例，女469例，年龄28～76岁，平均(53.7±14.2)岁。实施诊间预约前后患者的一般资料比较，差异无统计学意义(P>0.05)，具有可比性。

我院2017年10月前有114电话、北京市统一挂号平台网、自助机预约、窗口预约4种预约挂号方式，门诊预约率持续在15%以下。为了改善群众就医感受、解决“看病难”门诊部开展了经改进创新的诊间预约挂号模式。, http://www.100md.com(周晓峰 王小竹 王永梅 狄慧苗 吕宝霞 刘丹)
