四维彩超在产前胎儿畸形筛查中的应用价值(1)

     [摘要] 目的 观察和分析四维彩超在产前诊断胎儿畸形的临床价值。方法 方便选取该院2016—2017年 进行四维彩超检查的孕妇,诊断随访32例发育异常胎儿,并对胎儿进行常规二维超声检查,对疑似发育异常的胎儿采取3D成像和记录的检测手段,采集胎儿立体和运动图像等检测数据,获得形态及行为学影像资料,存入工作站,检查完后自然回放图像与四维彩超图像对比,据此做出诊断。结果 发现胎儿畸形32例45处,二维超声诊断与产后诊断相符的有33处(符合率73.3%),四维彩超诊断43处(符合率95.6%),两者比较差异有统计学意义(P<0.05)。结论 通过四维彩超能够实时直观地显示胎儿各部位解剖结构的动态三维立体图像,比传统的二维超声提供更丰富的图像信息,值得临床推广应用。

    [Abstract] Objective To observe and analyze the clinical value of four-dimensional color Doppler in screening of fetal malformation before delivery. Methods Convenient selection the delivery women with four-dimensional color Doppler examination in our hospital from 2016 to 2017 were selected and 32 cases of fetuses with abnormal growth were followed up, and the fetuses were given the routine two dimensional ultrasound examination, and the fetuses suspected with abnormal growth adopted the 3D imaging and the test means were recorded and the shape and behavior were obtained and stored into the work station, after examination, the image was naturally replayed and compared with the four-dimensional color Doppler images. Results Of 32 cases of fetuses, there were 45 fetal malformations, and the coincidence rate of two dimensional ultrasound diagnosis and postpartum diagnosis was 73.3%(33), and the coincidence rate of two dimensional ultrasound diagnosis and four-dimensional color Doppler examination was 95.6%(43), and the difference was Statistically sighificaont(P<0.05). Conclusion The four-dimensional color Doppler can show the dynamic three-dimensional stereo-picture of fetal anatomic structure in various sites in real time, which can provide richer image information compared with those of the traditional two-dimensional ultrasound, and it is worth clinical promotion and application.

    二维超声可作为一种诊断方式,可以用与早期对胎儿畸形的发现和诊断[1]。但是从目前的临床研究发现若想要获取平面图像可以采用二维超声,但是如果在诊断胎儿病情时比较复杂,单纯依靠平面图像无法将病情诊断清楚,那么就需要采用更加先进的扫描技术,目前随着彩超技术的不断发展,四维彩超技术可以提供多角度全方位的图像信息,在胎儿畸形诊断方面具有更高的诊断价值[2-3]。方便选取该院2016—2017年 进行RT-3DU检查的孕妇,诊断随访了32例发育异常胎儿,現报道如下。

    1 资料与方法

    1.1 一般资料

    方便选取该院需要做RT-3DU检查的孕妇,并且在诊断诊断后随访32例发育异常胎儿。孕妇的平均年龄为(28.3±2.3)岁,妊娠22~38周,平均(28.98±2.95)周。

    1.2 方法仪器

    应用GE Voluson E8超声仪,将探头频率设置为合理范围,一般为3.5~5.5 MHz,经腹容积探头的频率设置为3.5~5.5 MHz。在检查之前让孕妇采取仰卧位,先对胎儿采取常规二维超声检查(two dimensional ultrasonography,2DU),在二维检查完成后进行RT-3DU。选择实时三维(real-time three dimension,RT-3D)状态并实时调整立体数据箱大小,屏幕上若出现动态立体图像,可将探头缓慢移动并对胎儿异常的部位做进一步的跟踪扫查,采集图像后对RT-3D图像进行分析。, http://www.100md.com(吴海云 王冬冰)
