阻生齿拨除中应用微创拔牙技术的有效性及优势研究(1)

     [摘要] 目的 探讨阻生齿拨除中应用微创拔牙技术的有效性及优势。方法 按照随机数字表法将方便选取2016年1月—2017年2月该院诊治的84例阻生齿拔除患者分组,每组42例,对照组用传统的拔牙器械进行拔牙,观察组应用微创拔牙技术(采用高速涡轮钻以及微创拔牙刀),比较两组阻生齿拔除效果;阻生齿拔除所需的时间、术中出血量、手术后平均愈合时间;术中和术后不同时间点患者疼痛水平;张口困难、邻牙松动、干槽症和出血等并发症发生率。结果 观察组总有效率92.86%;对照组总有效率76.19%(P<0.05)。观察组术中、术后2天和1周疼痛水平(6.25±0.21)分、(3.42±0.71)分和(1.02±0.66)分低于对照组,阻生齿拔除所需的时间、术中出血量、手术后平均愈合时间(33.51±4.41)min、(25.51±2.61)mL、(7.62±1.21)d低于对照组(P<0.05)。观察组并发症发生率7.14%低于对照组23.81%(P<0.05。结论 阻生齿拨除中应用微创拔牙技术的有效性高,优势在于创伤小,出血少,并发症少,愈合时间快,值得推广。

    [Abstract] Objective To study the effectiveness and advantages of minimally invasive extraction technology in removing the impacted teeth. Methods 84 cases of patients with impacted teeth removal from January 2016 to February 2017 were convenient selected and randomly divided into two groups with 42 cases in each, the control group applied the traditional extraction device, while the observation group applied the minimally invasive extraction technology (high speed turbine drill and minimally invasive extraction knife), and the extraction effect, extraction time, intraoperative bleeding amount, average healing time after surgery, pain levels at different time points in operation and after operation, incidence rate of complications were compared between the two groups. Results The total effective rate in the observation group and in the control group was respectively 92.86% and 76.19%(P<0.05), and the pain levels in operation, 2 d and 1-week after surgery in the observation group were respectively (6.25±0.21)points, (3.42±0.71)points and (1.02±0.66)points, which were lower than those in the control group, and the extraction time of impacted teeth, intraoperative bleeding amount and average healing time after surgery were respectively (33.51±4.41)min, (25.51±2.61)mL, (7.62±1.21)d, which were lower than those in the control group(P<0.05), and the incidence rate of complications in the observation group was lower than that in the control group, (7.14% vs 23.81%)(P<0.05). Conclusion The effectiveness of minimally invasive extraction technology in removing the impacted teeth is high, with small trauma, less hemorrhage, few complications, and rapid healing time, and it is worth promotion.

    在臨床中,阻生齿拔除术操作难度比较大,在牙科手术中操作具有较大复杂性,其拔除主要遵循的原理是轮轴原理、杠杆原理和锲力原理[1]。传统多通过骨凿劈冠凿骨分根的方法,但创伤大,由于多种手术器械的应用和敲打时较大的响声和振动声,术中患者恐惧感强,牙齿拔除难度大,术后容易出现面颊肿胀和拔牙窝疼痛,甚至出现牙槽骨骨折、颌骨骨折和下齿槽和神经损伤,可给患者带来痛苦。该研究按照随机数字表法将2016年1月—2017年2月84例阻生齿拔除患者分组,每组42例,分析了阻生齿拨除中应用微创拔牙技术的有效性及优势,现报道如下。, 百拇医药(蒋建平)
