MRI结合MRCP在胆囊癌与胆囊腺肌症鉴别诊断中的应用价值(1)

     [摘要] 目的 MRI結合MRCP在胆囊癌与胆囊腺肌症鉴别诊断中的应用价值。方法 方便选取该院2017年10月—2018年10月收治的54例胆囊癌患者和54例胆囊 腺肌症患者,应用MRI进行鉴别,设为对照组。观察组MRI扫描过程与对照组相同,额外应用MRCP诊断,采用加强序列法,通过静脉通道给予Gd-DTPA滴注,剂量方面控制在0.3~0.4 mL/kg,行T1横断扫描、矢状面扫描,以冠位成像,由医疗人员对影像资料进行重组和分析。对比两组鉴别的准确率、特异性、灵敏性、鉴别时间。 结果 观察组鉴别准确率95.37%、漏诊率2.78%、误诊率1.85%、准确率95.37%、特异性102、灵敏性104、时间(2.7±0.3)h。对照组鉴别准确率77.78%、漏诊率8.33%、误诊率13.89%、准确率77.78%、特异性87、灵敏性81、时间(1.6±0.2)h。两组相比差异有统计学意义(t=6.004、5.987、6.307,P<0.05)。 结论 在胆囊癌与胆囊腺肌症鉴别诊断中。应用MRI结合MRCP法可提升鉴别准确性,可作为胆囊癌与胆囊腺肌症鉴别诊断的首选方式。

    The Value of MRI Combined with MRCP in Differential Diagnosis of Gallbladder Carcinoma and Gallbladder Adenomyosis

, 百拇医药     [Abstract] Objective The value of MRI combined with MRCP in the differential diagnosis of gallbladder carcinoma and gallbladder adenomyosis. Methods A total of 54 patients with gallbladder carcinoma and 54 patients with gallbladder adenomyosis were convenient enrolled in our hospital from October 2017 to October 2018. The patients were identified by MRI as the control group. The MRI scan process of the observation group was the same as that of the control group, while the MRCP diagnosis was additionally applied. The Gd-DTPA instillation was administered by the venous channel by the enhanced sequence method. The dose was controlled at 0.3~0.4 mL/kg, and the T1 transverse scan and sagittal scan were performed. The image was reconstructed and analyzed by medical personnel with crown imaging. The accuracy, specificity, sensitivity and time of identification of the two groups were compared. Results The accuracy of the observation group was 95.37%, the rate of missed diagnosis was 2.78%, the misdiagnosis rate was 1.85%, the accuracy rate was 95.37%, the specificity was 102, the sensitivity was 104, and the time was (2.7±0.3) h. The accuracy of the control group was 77.78%, the rate of missed diagnosis was 8.33%, the misdiagnosis rate was 13.89%, the accuracy rate was 77.78%, the specificity was 87, the sensitivity was 81, and the time was (1.6±0.2) h. The difference between the two groups was statistically significant(t=6.004, 5.987, 6.307,P<0.05). Conclusion In the differential diagnosis of gallbladder carcinoma and gallbladder adenomyosis.
