老年射血分数保留心力衰竭患者血清胱抑素C水平变化及其意义(3)
HFpEF最重要的病理生理机制是舒张功能障碍，超声心动图是目前临床上唯一可用于评估舒张功能不全的影像学检查方法，其主要的舒张功能异常指标包括E/A、E′、E/E′等。该研究显示HFpEF组LAD[(35.48±4.87)mm]、E′[(5.07±1.31)cm/s]、E/E′(15.71±4.56)均高于对照组LAD[(31.27±3.63)mm]、E′[(5.77±1.44)cm/s]、E/E′(13.88±4.12)(P<0.05)。HFpEF血清Cys-C与E′、E/A比值呈负相关(r=-0.2、-0.167，P<0.05)。推测血清Cys-C有助于舒张功能不全的诊断。GXu等[9]研究了381名老年HFpEF患者，发现血清Cys-C与E/A呈负相关(r=-0.224，P<0.05)，与该研究结果一致。
综述所述，血清Cys-C水平增高与老年射血分数保留心力衰竭患者的发生有关，可反映其嚴重程度。联合检测血清Cys-C、NT-proBNP、超声心动图有助于老年患者HFpEF的早期诊断，具有重要的临床价值。但该研究样本量较小，有待大样本量的临床研究进一步明确血清Cys-C的临床价值。
