超声在评估剖宫产后子宫瘢痕部位妊娠中的应用(1)

     [摘要] 目的 探討超声应用于剖宫产后瘢痕妊娠诊断评估的临床价值。 方法 回顾性分析2015年4月—2019年3月期间,于该院接受治疗的124例疑似瘢痕妊娠患者临床资料,以术后组织学病理为“金标准”,分析灰阶B超、二维彩超、三维彩超诊断瘢痕妊娠的准确性。 结果 经术后组织学病理检查,专家会诊考虑为瘢痕妊娠96例(77.42%)与非瘢痕妊娠28例(22.58%)。4种查方法诊断瘢痕妊娠的阳性预测值、特异度比较差异无统计学意义(χ2=3.219,P=0.200;χ2=3.930,P=0.140),三维彩超阴性预测值、灵敏度均明显高于灰阶B超,差异有统计学意义(χ2=6.753,P=0.034;χ2=6.366,P=0.041),三维彩超准确度明显高于灰阶B超、二维彩超,差异有统计学意义(P=0.009),且κ值也均较高。 结论 不同超声模式对剖宫产后瘢痕部位妊娠有不同的诊断效能,其中三维彩超更具优势,有利于早期诊断评估瘢痕妊娠并指导临床治疗。

    Application of Ultrasound in the Evaluation of Pregnancy in Uterine Scar after Cesarean Section

    [Abstract] Objective To investigate the clinical value of ultrasound in the diagnosis and evaluation of scar pregnancy after cesarean section. Methods The clinical data of 124 patients with suspected scar pregnancy who were treated in the hospital from April 2015 to March 2019 were retrospectively analyzed. The postoperative histological pathology was regarded as the “gold standard”, and the gray scale B-ultrasound and two-dimensional were analyzed. Color Doppler ultrasound, three-dimensional color Doppler ultrasound diagnosis of scar pregnancy accuracy. Results After histological pathological examination, 96 cases (77.42%) of scar pregnancy and 28 cases (22.58%) of non-scarring pregnancy were considered. There were no significant differences in the positive predictive value and specificity between the four ultrasound examinations for the diagnosis of scar pregnancy,the difference was not statistically significant (χ2=3.219, P=0.200; χ2=3.930, P=0.140). The three-dimensional color ultrasound negative predictive value and sensitivity were significantly higher than gray-scale B-ultrasound ,the difference was statistically significant(χ2=6.753, P=0.034; χ2=6.366, P=0.041), the accuracy of three-dimensional color Doppler ultrasound is significantly higher than gray-scale B-ultrasound, two-dimensional color Doppler ultrasound,the difference was statistically significant(P=0.009), and κ value is also high. Conclusion Different ultrasound modes have different diagnostic efficacy for scar pregnancy after cesarean section.
