高频彩色多普勒超声在早期诊断乳腺癌腋窝淋巴结转移中的价值分析(1)

     [摘要] 目的 探究高頻彩色多普勒超声应用于早期乳腺癌腋窝淋巴结转移的诊断作用分析。方法 从该院2015年1月—2018年12月被该院病理诊断为乳腺癌患者中方便选出50例,患者予以手术治疗前均进行高频彩色多普勒超声检查,并以术后病理学检查结果为诊断的对比标准,将病理诊断结果为腋窝淋巴结转移者作为腋窝淋巴结转移组(转移组),其余为未发生腋窝淋巴结转移组(即未转移组),对术前高频彩色多普勒超声诊断腋窝淋巴结转移的诊断学价值指标进行评价。结果 相比未转移组皮质向心性增厚为13.04%(3/23)、淋巴结内钙化灶为4.35%(1/23)及淋巴结的边界不清检出率为17.39%(4/23),转移组皮质向心性增厚为66.67%(18/27)、淋巴结内钙化灶为59.26%(16/27)及淋巴结的边界不清检出率为70.37%(19/27)较高 (χ2=4.038、16.689、14.034,P<0.05)。转移组患者血流分布以周边型为主所占比例为62.96%(17/27),血流信号分级以3级占74.07%(20/27)为主;未转移组血流分布中门型所占比例为60.87%(14/23)在所有分型中所占比例最高,血流信号以分级2级为主所占比例为65.22%(15/23)(P<0.05)。高频彩超检查的灵敏度、特异度、漏诊率、误诊率分别为85.19%、69.56%、14.81%以及30.43%。结论 高频彩超可以用于判断乳腺癌腋窝早期是否存在淋巴结转移中,具有良好的应用价值,适宜临床应用。

    Value of High Frequency Color Doppler Ultrasonography in Early Diagnosis of Axillary Lymph Node Metastasis in Breast Cancer

    [Abstract] Objective To investigate the diagnostic effect of high frequency color Doppler ultrasound in the diagnosis of axillary lymph node metastasis in early breast cancer. Methods From January 2015 to December 2018, 50 patients with pathological diagnosis of breast cancer were conveniently selected. All patients underwent high-frequency color Doppler ultrasonography before surgical treatment, and postoperative pathological examination. The results were the diagnostic criteria. The pathological diagnosis was axillary lymph node metastasis as axillary lymph node metastasis group (metastasis group), and the rest were no axillary lymph node metastasis group (ie, non-metastasis group). Preoperative high-frequency color Doppler diagnostic value of ultrasound diagnosis of axillary lymph node metastasis was evaluated. Results It showed that the concentric thickening of the non-metastatic group was 13.04% (3/23), the calcification in the lymph nodes was 4.35% (1/23), and the detection rate of the lymph node was 17.39% (4/23). The centripetal thickening of the metastatic group was 66.67% (18/27), the calcification in the lymph nodes was 59.26% (16/27), and the detection rate of the lymph nodes was 70.37% (19/27)(χ2=4.038, 16.689, 14.034, P<0.05). The distribution of blood flow in the metastatic group was 62.96% (17/27), and the blood flow signal was graded at 74.07% (20/27). The blood flow distribution in the non-metastasis group was phenotype proportion of 60.87% (14/23) was the highest among all types, and the proportion of blood flow signal in grade 2 was 65.22% (15/23) (P<0.05). The sensitivity, specificity, missed diagnosis rate and misdiagnosis rate of high frequency color Doppler ultrasound examination were 85.19%, 69.56%, 14.81% and 30.43%, respectively. Conclusion High-frequency color Doppler ultrasound can be used to determine whether there is lymph node metastasis in the early stage of breast cancer, which has good application value and is suitable for clinical application., http://www.100md.com(苏佩相 罗博哲)
