[摘要] 目的：强调多功能(核酸荧光染色+半导体激光+流式细胞技术)全自动五分类血细胞分析仪在细胞形态学检查中仍然也只能作为一种过筛手段，尤其是在仪器报警异常的病理条件下，必须进行人工细胞形态学镜检复查的重要性。方法：在生物显微镜油镜下对Sysmex XS-1000i血细胞分析仪报警异常的1 080例血象进行涂片染色做细胞形态学检查和人工白细胞分类，然后对与疾病相关的主要异常细胞学形态分组作统计学分析。结果：Sysmex XS-1000i血细胞分析仪报警异常的1 080例血象复检，782例可检出异常的细胞形态(真阳性)，检出率(真阳性率)为72.40%。结论：Sysmex XS-1000i血细胞分析仪对报警异常血象只能起到初筛的目的，不能代替人工细胞形态学复检，只有二者结合，优势互补，才能减少漏诊和误诊。

The Microscopic examination of abnormal blood pictures displayed by Sysmex XS-1000i

[Abstract] Objective: To show the importance of manual morphological examination review when there are abnormal pathological conditions. The Sysmex XS-1000i hematology analyzer can only be used as a screening tool in cell morphology examination. Methods: The oil microscope was used in the observation of cell morphology and manual WBC differential analysis in 1 080 cases which were shown abnormal in the Sysmex XS-1000i hematology analyzer, then the main disease-related form of abnormal cytology group were statistically analyzed. Results: 782 cases of abnormal cell morphology can be detected (true positive) in the recheck of 1 080 cases. The detection rate (true positive rate) was 72.40%. Conclusion: The manual morphological examination can not be replaced by Sysmex XS-1000i hematology analyzer especially when there are abnormal blood pictures. Both methods should be used in order to alleviate mistakes and missed diagnosis.

近年来虽然各种类型的血细胞分析仪得以广泛应用，帮助检验人员快速完成大量血标本的检测，大大提高了工作效率，但迄今为止血细胞分析仪在形态学检查中仍只能作为一种过筛手段，当遇到可疑情况，尤其是在病理条件下，必须进行人工镜检复查，这已是不争的事实[1-2]。如何能够让血细胞分析仪更好地为临床服务，一直是各国血液学专家共同关注的问题[2-6]。2005年国际血液学复检专家组通过对13 298份血标本进行详细分析后，推荐了41条自动血细胞分析和分类复检规则[5-6]。但这只是通则，对于不同仪器或不同实验室的病源，由于筛选目的不同，其并无针对性和特异性。本科利用Sysmex XS-1 000i血细胞分析仪的IP信息报警系统对WBC、RBC、PLT的数值异常和形态学异常的提示，进行了1 080例血涂片细胞形态学复检。
1 材料与方法

Sysmex XS-1000i血细胞分析仪报警异常的1 080例血象标本均来自本院检验科临检血液室2008年2月1日~ 2010年1月31日就诊做血常规的门诊及住院患者 ......
