高尿酸血症对IgA肾病的影响研究(1)
[摘要]目的 了解IgA肾病合并高尿酸血症患者的临床指标和病理特点，探讨高尿酸血症对IgA肾病的影响。方法 收集2013年3月～2017年6月在我院肾内科住院的经肾穿刺活检病理确诊为原发性IgA肾病的125例患者作为研究对象，其中男44例，女81例，根据血尿酸水平将患者分为血尿酸正常组(69例)与高尿酸血症组(56例)。记录患者的年龄、性别、血尿酸、血肌酐、24 h尿蛋白定量、血清清蛋白、血胆固醇、血三酰甘油、血红蛋白等指标，所有患者进行肾脏病理检查并行Lee分级，统计分析两组的临床和病理特点。结果 高尿酸血症组24 h尿蛋白定量、血肌酐、三酰甘油、总胆固醇水平高于血尿酸正常组，差异有统计学意义(P<0.05)。高尿酸血症组患者肾脏病理Lee分级情况较血尿酸正常组差(P<0.05)。结论 IgA肾病患者较易发生高尿酸血症，与血尿酸正常者相比，临床表现和肾脏病理损伤更为严重，提示高尿酸血症与IgA肾病预后相关，应予重视并及时有效地进行干预治疗。

[Abstract]Objective To understand the clinical indicators and pathological features of patients with IgA nephropathy and hyperuricemia，and to investigate the influence of hyperuricemia on IgA nephropathy.Methods A total of 125 patients with primary IgA nephropathy definitely diagnosed by pathological detection after renal biopsy who hospitalized in nephrology department of our hospital from March 2013 to June 2017 were enrolled.Among them，the ratio of male to female was 44 cases to 81 cases.According to the serum uric acid level，they were divided into the normal uric acid group (69 cases) and the hyperuricemia group (56 cases).The indexes of age，gender，serum uric acid，serum creatinine，24-hour urinary protein，serumalbumin，blood cholesterol，triglyceride and hemoglobin were recorded.All patients were performed with renal pathological examinations and Lee parallel grading.The clinical and pathological features of the two groups were statistically analyzed.Results The level of 24-hour urinary protein excretion，serum creatinine，triglyceride and total cholesterol in the hyperuricemia group were higher than those in the normal uric acid group (P<0.05).The Lee grading of renal pathology in the hyperuricemia group was worse than that in normal uric acid group (P<0.05).Conclusion Patients with IgA nephropathy are likely to develop hyperuricemia.Compared with patients with normal serum uric acid，their clinical manifestations and renal pathological injury are more serious，suggesting that hyperuricemia is associated with the prognosis of IgA nephropathy and should be paid great attention and provided with effective treatment as an intervention in time.
