UF-1000i尿沉渣分析仪在真菌性尿路感染中的应用(1)

     【摘要】 目的:探讨UF-1000i尿沉渣分析仪的真菌计数在真菌性尿路感染中的应用价值。方法:将237份无菌操作留取的中段尿标本进行真(细)菌培养鉴定,并在UF-1000i尿沉渣分析仪上进行真菌数量的检测和人工显微镜镜检,分析UF-1000i尿沉渣分析仪的诊断价值。结果:真菌培养阳性31例,以真菌培养结果为金标准,UF-1000i尿沉渣分析仪真菌计数AOC曲线下面积为0.997时,真菌最佳诊断临界值为85.10个/μl, 用该值诊断尿路真菌感染的灵敏度为0.9678,特异度为0.9757,阳性预测值为0.8571,阴性预测值为0.9950;当诊断临界值为161.55个/μl时,诊断尿路真菌感染的特异度为100%,以此11.34%(27/237)的样本可以快速初筛为真菌性尿路感染;当临界值为30.15个/μl时,诊断尿路感染的阴性预测值为100%,以此83.54%(198/237)的样本可以快速排除真菌性尿路感染;人工显微镜检查23例阳性标本也为UF-1000i检测阳性。结论:UF-1000i尿沉渣分析仪检测真菌可作为临床真菌性尿路感染的快速筛检,结合人工显微镜检查可以快速确诊真菌性尿路感染。

    【Abstract】 Objective:To evaluate the parameter of yeast for urine flow cytomerer Sysmex UF-1000i in urinary tract infections (UTI). Method:A total of 237 specimens were analyzed and then compared with urine culture. Result:The area under the receiver operating curve (AOC) was 0.997 for yeast in UF-1000i and the best cutoff value obtained by AOC analysis was85.10 yeast/μl, analytic parameters such as sensitivity, specificity, positive predictive value, negative predictive value, were 0.9678, 0.9757,0.8571,and 0.9950, respectively. When the cutoff value obtained was 161.55 yeast/μl, the specificity was 100% and 11.34% samples were diagnosed for fungal UTI. Furthermore, the cutoff value was 30.15 yeast/μl, the negative predictive value was 100% and 83.54% samples could be ruled out fungal UTI. Conclusion:The parameter of yeast detected by UF-1000i urine sediment analysis can be used as a rapid screening of clinical fungal UTI, combined with manual microscopic examination can be quickly diagnosed with fungal UTI ......
