不同麻醉方法对下肢骨折手术患者的循环功能影响对比(1)

     【摘要】 目的:探究不同麻醉方式对下肢骨折手术中患者循环功能的影响。方法:选取2012年9月-2013年5月本院住院治疗的90例下肢骨折手术患者,按照麻醉方法分为两组,全麻组45例,联合组45例,分别采用全麻与硬膜外麻醉联合腰麻方式进行麻醉。分别在麻醉前(T0)、麻醉后(T1)、手术进行5 min(T2)、15 min(T3)、30 min(T4)、60 min(T5)对患者的平均血压(MAP)、心率(HR)、呼吸频率(RR)、血氧浓度(SpO2)指标数值的变化进行比较。结果:两组在术中MAP与T0时相比均有明显变化,全麻组术前MAP为(108.1±8.8)mm Hg,最低为(88.3±4.2)mm Hg,联合组术前MAP为(107.9±9.2)mm Hg,最低为(103.5±3.7)mm Hg,全麻组变化较为显著,与联合组比较差异有统计学意义(P<0.05);HR全麻组变化从(85.2±4.6)次/min到(65.8±5.3)次/min,联合组(87.6±3.2)次/min到(73.3±4.6)次/min,全麻组变化明显,差异有统计学意义(P<0.05);两组患者SpO2与RR变化比较差异均无统计学意义(P>0.05)。结论:硬膜外麻醉联合腰麻在老年下肢骨折手术中各项指标变化相对较少,麻醉效果好,安全性更高,对于循环功能影响更小,且临床疗效较为显著。

    【Abstract】 Objective:To study and analyze the impacts of different anesthesia methods on the circulatory function of elderly patients during lower limb fracture surgery.Method:90 patients with lower limb fracture admitted in our hospital from September 2012 to May 2013 were retrospectively analyzed.They were divided into general anesthesia group and CSEA group and given general anesthesia, CSEA, separately.MAP, HR, RR and SPO2 at T0 (before anesthesia), T1 (after anesthesia), T2 (operation was performed for 5 min), T3 (operation was performed for 15 min), T4 (operation was performed for 30 min) and T5 (operation was performed for 60 min) of the two groups were compared.Result:In general anesthesia group, the change of MAP was (108.1±8.8)mm Hg, (88.3±4.2) mm Hg and in CSEA group was (107.9±9.2) mm Hg, (103.5±3.7) mm Hg, the change in the former was more than that in the latter, the difference was statistically significant (P<0.05).In general anesthesia group, the change of HR was (85.2±4.6)times/min,(65.8±5.3) times/min and in CSEA group was (87.6±3.2) times/min,(73.3±4.6) times/min, the changes in the former were much more than those in the latter, the difference was statistically significant (P<0.05).The differences in SpO2 and RR between groups were not statistically significant (P>0.05).Conclusion:CSEA for lower limb fracture surgery of elderly patients has little change in each index during the operation; it has good anesthetic effect with little impact on the circulatory function of patients, so it’s more suitable to be applied in lower limb fracture surgery of elderly patients with higher safety.
