细节护理在儿童肱骨髁上GartlandⅡ、Ⅲ型骨折行手法整复配合中药熏洗中的应用体会(1)
【摘要】 目的：探讨细节护理在儿童肱骨髁上GartlandⅡ、Ⅲ型骨折行手法整复配合中药熏洗中的应用。方法：选取2013年5月-2014年5月本院收治的肱骨髁上GartlandⅡ、Ⅲ型骨折患儿80例作为研究对象，采用随机数字表法将其分为干预组和对照组，每组40例，所选患儿均接受手法整复配合中药熏洗治疗。对照组给予骨科常规护理，干预组在对照组的基础上给予细节护理，观察比较两组疼痛程度、手法整复复位配合程度、术后并发症发生情况、家长满意度及护理达标情况。结果：干预组OPS评分及手法整复复位总配合率高于对照组，比较差异均有统计学意义(P<0.05)；干预组术后并发症发生情况、家长满意度及护理达标情况均明显优于对照组，比较差异均有统计学意义(P<0.05)。结论：细节护理可有效减轻肱骨髁上GartlandⅡ、Ⅲ型骨折患儿疼痛，提高患儿手法整复复位配合度，减少术后并发症发生，提高家长满意度，可广泛应用于临床。

【Abstract】 Objective：To investigate the application of detailed nursing in the manipulation combined with herbal fumigation treatment for children with humeral supracondylar fracture of Gartland type Ⅱ，Ⅲ.Method：80 cases of children with humeral supracondylar fracture of Gartland type Ⅱ，Ⅲ in our hospital from May 2013 to May 2014 were selected and randomly divided into the interventional group and the control group，with each group 40 cases.All patients accepted the treatment of manipulation combined with herbal fumigation.Children in the control group were given routine nursing of orthopedics，the interventional group was given detailed nursing on the basis of the control group.The pain degree，the cooperation degree of manipulation reduction，the incidence of postoperative complications and the situation of parents' satisfaction and nursing standards were observed and compared between the two groups.Result：The OPS score and the cooperation degree of manipulation reduction in the interventional group were higher than those in the control group，the differences were statistically significant(P<0.05).The incidence of postoperative complications and the situation of parents' satisfaction and nursing standards in the interventional group were better than those in the control group，the differences were statistically significant(P<0.05).Conclusion：Detailed nursing can effectively reduce the pain degree of children with humeral supracondylar fracture of Gartland type Ⅱ，Ⅲ，improve the cooperation of manipulation reduction，reduce the incidence of postoperative complications and improve the satisfaction of parents.It can be widely applied to clinic.

肱骨髁上骨折在儿童骨折上较为多见，主要由运动损伤、生活损伤、交通事故等造成，是指间接暴力导致肱骨干与肱骨髁交界处发生骨折损伤[1]。Gartland Ⅰ型肱骨髁上骨折采取夹板固定、石膏固定2～4周内即可取得较好恢复效果，而对于Gartland Ⅱ、Ⅲ型肱骨髁上骨折则大多需行手术治疗[2]。本院对Gartland Ⅱ、Ⅲ型肱骨髁上骨折采取手法整复复位联合克氏针内固定，同时配合中药熏洗治疗，取得较满意的效果，但由于患儿对治疗的配合能力较差，若护理不得当，亦有可能对患儿术后康复造成不良影响，导致术后并发症发生等不良预后。因此，为提高患儿对治疗的配合程度，避免各种潜在的危险因素，提高患儿家长满意度和手术护理质量等，本院对肱骨髁上GartlandⅡ、Ⅲ型骨折患儿采取细节护理干预，取得满意效果，现具体报道如下。, 百拇医药(钟丽娟 钟丽琼 郑丽芳)
