优质护理干预对维持性血液透析患者内瘘血管的影响(1)
【摘要】 目的：探讨优质护理干预对维持性血液透析患者内瘘血管的影响。方法：选取本院在2013年1月-2016年12月收治的维持性血液透析患者110例，根据随机数字表法将其分为观察组和对照组，每组各55例。对照组给予常规护理，观察组在此基础上给予优质护理干预，对比两组患者并发症发生率，内瘘闭塞率和动脉瘤样扩张率，采用SF-36生活质量量表评定患者护理后生活质量，并比较两组患者护理满意度。结果：经护理干预后，观察组患者出血、感染及血栓形成的并发症总发生率为7.27%，显著低于对照组的25.45%(P<0.05)；观察组患者内瘘闭塞率和动脉瘤样扩张率分别为1.82%、7.27%，显著低于对照组的14.55%、18.18%(P<0.05)；观察组患者的各项生活质量评分均显著高于对照组(P<0.05)；观察组患者护理满意度明显高于对照组(P<0.05)。结论：对维持性血液透析患者给予优质护理干预，可显著减少并发症发生率，并降低内瘘闭塞率和动脉瘤样扩张率，明显改善患者生活质量，提高护理满意度，值得临床推广。

【Abstract】 Objective：To study the effect of high quality nursing intervention on fistula blood vessels in maintenance hemodialysis patients.Method：110 cases of maintenance hemodialysis admitted in our hospital from January 2013 to December 2016 were selected，they were randomly divided into observation group and control group according to the random number table method，55 cases in each group.The control group was given routine care，and observation group was given high quality nursing intervention on this basis，the incidence of complications，fistula occlusion rate and aneurysm-like dilatation rate of two groups were compared，the SF-36 quality of life scale was used to assessd the patients’ care quality of life，and the nursing satisfaction of two groups were compared.Result：After nursing intervention，the total incidence of bleeding，infection and thrombosis complications in observation group was 7.27%，which was significantly lower than 25.45% of control group(P<0.05).The fistula occlusion rate and aneurysmal dilatation rate of observation group were 1.82% and 7.27% respectively，which were significantly lower than 14.55% and 18.18% of control group(P<0.05).The quality of life scores of observation group were significantly higher than those of control group(P<0.05).The nursing satisfaction of observation group was significantly higher than that of control group(P<0.05).Conclusion：The maintenance of patients with maintenance hemodialysis to give high quality nursing intervention can significantly reduce the incidence of complications，and reduce the fistula occlusion rate and aneurysm-like expansion rate，significantly improve the quality of life of patients，improve nursing satisfaction，worthy of clinical promotion.

維持性血液透析是终末期肾脏疾病患者利用血液透析或腹膜透析来挽救患者的生命，延长尿毒症患者生命的过渡方法[1]。近年来，随着各种疾病发病率的提高，高血压、糖尿病、慢性肾炎导致的尿毒症患者发病率也显著提高[2]。动静脉内瘘是血液透患者最理想的血管通路，可保证血液透析的充分性，提高患者生活质量，但长期透析下引发的动静脉内瘘功能丧失也成为了临床比较棘手的问题[3]。合理使用和保护血管通路，为患者提供优质护理服务，对于减少动静脉血管内瘘相关并发症，提高患者生活质量有积极影响[4]。本研究为探讨优质护理干预对维持性血液透析患者内瘘血管的影响，选取本院收治的维持性血液透析患者作为研究对象，效果确切，现报道如下。, 百拇医药(吴顺秀 熊佳静 文玲莉)
