自拟调胃升降汤治疗慢性萎缩性胃炎的临床疗效分析(1)
【摘要】 目的：探討自拟调胃升降汤治疗慢性萎缩性胃炎的临床疗效。方法：将2015年6月-2017年6月在本院中医科治疗的100例慢性萎缩性胃炎患者依据治疗方案的不同分为两组，每组50例。对照组采用常规西医治疗，观察组采用自拟调胃升降汤治疗，比较两组临床疗效、胃镜下征象变化、中医症候积分变化、幽门螺杆菌(Hp)根除率及复发率。结果：观察组总有效率为94.0%，明显高于对照组的76.0%，差异有统计学意义(P<0.05)；观察组治疗后胃黏膜苍白、黏膜颗粒样改变、黏膜皱襞变薄、黏膜血管透见、黏膜肠化结节等胃镜下征象积分明显低于对照组，差异均有统计学意义(P<0.05)；观察组治疗后胃脘痞闷刺痛、嗳气反酸、疲乏无力、食少纳呆等中医证候积分明显较对照组下降，差异均有统计学意义(P<0.05)；观察组治疗后Hp根除率为86.0%，复发率为12.0%，与对照组的70.0%、26.0%相比，差异均有统计学意义(P<0.05)。结论：自拟调胃升降汤治疗慢性萎缩性胃炎的临床疗效显著，能够有效保护胃黏膜，抑制肠上皮化生，减少固有腺体萎缩，减轻胃黏膜炎症，增强Hp清除效果，抑制复发，具有积极的临床意义。

【Abstract】 Objective：To investigate the clinical efficacy of self regulating stomach ascending and reducing decoction in the treatment of chronic atrophic gastritis.Method：A total of 100 patients with chronic atrophic gastritis treated in department of traditional Chinese medicine of our hospital from June 2015 to June 2017 were divided into two groups，50 cases in each group.According to different treatment plans，the control group was given routine treatment of Western medicine，the observation group was treated with Self Regulating Stomach Ascending and Reducing Decoction，the clinical efficacy，gastroscopic signs change，TCM symptom score change，Hp eradication rate and recurrence rate of two groups were compared.Result：The effective rate of the observation group was significantly higher than that of the control group(P<0.05).After treatment，gastric mucosal pallor，granular changes，mucosal thinning，mucosal vascular permeability，intestinal metaplasia nodules under gastroscope signs score of the observation group were significantly lower than those of the control group，the differences were statistically significant(P<0.05).After treatment，fullness in stomach pain，belching，acid regurgitation，fatigue，anorexia and other symptoms score of the observation group were significantly decreased compared with the control group，the differences were statistically significant(P<0.05).After treatment，the eradication rate of Hp of the observation group was 86.0%，the recurrence rate was 12.0%，compared with the control group’s 70.0%，26.0%，the differences were statistically significant(P<0.05).Conclusion：The clinical curative effect of chronic atrophic gastritis with Self Regulating Stomach Ascending and Reducing Decoction，can effectively protect the gastric mucosa，inhibition of intestinal metaplasia，reduce glands atrophy，alleviate the inflammation of gastric mucosa，enhance Hp removal effect，inhibition of recurrence，has positive clinical significance., 百拇医药(马志勇)
