美托洛尔对充血性心力衰竭QT离散度的影响研究(1)
【摘要】 目的：分析美托洛尔对充血性心力衰竭(CHF)QT离散度(QTd)的影响。方法：选取本院2015年8月-2017年8月收治的充血性心力衰竭患者242例为对象，根据不同的治疗方式将其分为对照组和观察组，各121例。对照组实施常规治疗，在此基础上，观察组联合美托洛尔进行治疗。观察比较伴与不伴室性心律失常者、不同心功能分级者以及两组治疗前后QTd水平。结果：伴有室性心律失常患者的QTd值为(78.26±8.14)ms，明显高于不伴室性心律失常患者的(65.33±9.18)ms，比较差异有统计学意义(t=6.145，P=0.000)；心功能Ⅱ级患者的QTd值为(68.15±10.18)ms，明显低于心功能Ⅲ、Ⅳ级患者的(79.32±13.14)ms，比較差异有统计学意义(t=3.886 8，P=0.000 0)；治疗后，观察组QTd值与对照组相比显著降低，差异有统计学意义(P<0.05)。结论：针对充血性心力衰竭患者采用美托洛尔治疗的效果显著，既可有效缩小患者QT离散度，且对于患者室性心律失常等具有积极防治作用，因此值得临床应用和推广。

【Abstract】 Objective：To analyze the effect of Metoprolol on QT dispersion of congestive heart failure(CHF).Method： A total of 242 patients with congestive heart failure treated in our hospital from August 2015 to August 2017 were selected as the study objects.According to different treatment methods，they were divided into control group and observation group，121 in each group.The control group was treated with conventional therapy，and on this basis，the observation group was treated with metoprolol.The levels of QTd before and after treatment in two groups， with and without ventricular arrhythmias，different heart function grades were observed and compared.Result：The QTd value of patients with ventricular arrhythmias was(78.26±8.14) ms，which was significantly higher than that of patients without ventricular arrhythmias(65.33 ±9.18) ms，the difference was statistically significant(t=6.145，P=0.000).The QTd value of patients with heart function Ⅱ grade was (68.15 ±10.18) ms，which was significantly lower than that of patients with heart function Ⅲ and Ⅳ grade(79.32±13.14)ms，the difference was statistically significant(t=3.886 8，P=0.000 0).After treatment，the QTd value in the observation group was significantly lower than that of the control group，the difference was statistically significant(P<0.05). Conclusion：The effect of Metoprolol treatment for patients with congestive heart failure is very significant，which can effectively reduce QT dispersion of patients，and has an active preventive and therapeutic effect on ventricular arrhythmias，so it is worthy of clinical application and promotion.
