肌内效贴联合强制性使用运动疗法治疗痉挛型偏瘫的临床应用(1)
【摘要】 目的：探讨肌内效贴联合强制性使用运动疗法(CIMT)在痉挛型偏瘫患儿中的临床应用效果及价值。方法：选取2017年1-6月本院儿科收治的80例痉挛型偏瘫患儿作为研究对象，将其按照随机数字表法分为对照组和观察组，各40例。两组均给予CIMT，观察组在此基础上给予肌内效贴。对比两组治疗3、6、9个月后粗大运动功能评分(GMFM-88)、偏瘫分级(Brunnstrom分级)、社会生活能力(婴儿-初中生社会生活能力量表即SM量表)、肌力和肌张力情况。结果：两组GMFM-88评分、Brunnstrom分级、SM评分、患侧上肢肌力分级、患侧下肢肌力分级均随着时间延长显著升高(P<0.05)，且治疗3、6、9个月后观察组GMFM-88评分、Brunnstrom分级、SM评分、患侧上肢肌力分级、患侧下肢肌力分级均高于对照组(P<0.05)；两组患侧上肢肌张力和患侧下肢肌张力分级均随着时间延长显著降低(P<0.05)，且治疗3、6、9个月后观察组患侧上肢肌张力和患侧下肢肌张力分级均低于对照组(P<0.05)。结论：肌内效贴联合CIMT能够显著改善痉挛型偏瘫患儿的粗大运动功能、偏瘫分级、患侧上下肢肌力和肌张力情况，并且还可提升其社会生活能力。

【Abstract】 Objective：To explore the clinical effect and value of kinesio taping combined with constraint-indaced movement therapy(CIMT) in children with spastic hemiplegia.Method：80 children with spastic hemiplegia from January to June 2017 were selected as the research objects，they were randomly divided into the control group and the observation group，40 cases in each group.The two groups were given CIMT，and the observation group was given intramuscular paste on this basis.The coarse motion function score(GMFM-88)，the classification of hemiplegia (Brunnstrom classification)，the social life ability(the infant-junior high school social life energy meter，the SM scale)，the muscle strength and the muscle tension condition were compared between the two groups after 3，6 and 9 months.Result：There were two groups of GMFM-88 scores，Brunnstrom classification，SM scores，upper limb muscle strength classification and lower limb muscle strength increased significantly with time(P<0.05)；and 3，6，9 months after treatment，GMFM-88 scores，Brunnstrom classification，SM scores，upper limb muscle strength classification and lower limb muscle strength of the observation group were higher than those of the control group(P<0.05).Grade of muscle tension of upper limb and lower extremity in two groups decreased significantly with time(P<0.05)， and 3，6，9 months after treatment，grade of muscle tension of upper limb and lower extremity in the observation group were lower than those in control group(P<0.05).Conclusion：Kinesio taping combined with CIMT can significantly improve gross motor function，hemiplegia grade，upper and lower limbs muscle strength and muscle tension in children with spastic hemiplegia，and also improve their social life ability., http://www.100md.com(刘诗贤 陈福建)
