无创机械通气联合枸橼酸咖啡因治疗早产儿呼吸暂停的效果观察(1)

     【摘要】 目的:探究早產儿呼吸暂停采取无创机械通气(NIPPV)联合枸橼酸咖啡因的临床疗效。方法:选取2016年2月-2019年2月在本院治疗的早产儿呼吸暂停患儿共75例,其中2018年1月-2019年2月50例。按治疗方法分成对照组A(n=25)、对照组B(n=25)与观察组(n=25)。对照组A行持续气道正压(CPAP)联合枸橼酸咖啡因治疗,对照组B行NIPPV治疗,观察组行NIPPV联合枸橼酸咖啡因治疗,比较三组监测指标、临床疗效及并发症。结果:观察组呼吸暂停次数,呼吸暂停持续天数、用氧时间、住院时间及无创呼吸时间均低于对照组A、B,且治疗总有效率高于对照组A、B(P<0.05)。观察组慢性肺病、颅内出血与视网膜病变发生率均低于对照组A、B(P<0.05)。观察组脑白质软化发生率略低于对照组A、B,但差异均无统计学意义(P>0.05)。观察组鼻黏膜损伤发生率略高于对照组A,而略低于对照组B,差异均无统计学意义(P>0.05)。结论:NIPPV联合枸橼酸咖啡因治疗早产儿呼吸暂停可减少每日呼吸暂停次数,缩短无创呼吸时间、用氧时间与住院时间,改善血气指标,提高临床疗效,减少并发症发生率。

    [Abstract] Objective: To explore the clinical effect of non-invasive positive pressure ventilation (NIPPV) combined with Caffeine Citrate in the treatment of apnea in premature infants. Method: A total of 75 cases of premature infants with apnea treated in our hospital from February 2016 to February 2019 were selected, included 50 cases from January 2018 to February 2019. According to the treatment method, they were divided into control group A (n=25), control group B (n=25) and observation group (n=25). The control group A was treated with continuous positive airway pressure (CPAP) combined with Caffeine Citrate, the control group B was treated with NIPPV, the observation group was treated with NIPPV combined with Caffeine Citrate. The monitoring indicators, clinical efficacy and complications of the three groups were compared. Result: The number of apnea, the duration of apnea, oxygen use time, hospitalization time and non-invasive breathing time in the observation group were lower than those in the control group A and B, the total effective rate was higher than that in the control group A and B (P<0.05). The incidence of chronic pulmonary disease, intracranial hemorrhage and retinopathy in the observation group were lower than those in the control group A and B (P<0.05). The incidence of leukomalacia in the observation group was slightly lower than that in the control group A and B, but there was no significant difference (P>0.05). The incidence of nasal mucosal injury in the observation group was slightly higher than that in the control group A and slightly lower than that in the control group B, but there were no significant differences (P>0.05). Conclusion: NIPPV combined with Caffeine Citrate in the treatment of apnea in premature infants can reduce the number of daily apnea, shorten the non-invasive breathing time, oxygen time and hospitalization time, improve blood gas indicators , clinical efficacy and reduce the incidence of complications., 百拇医药(詹志义 刘宁 钟维维)
