儿童先天性白内障术后黄斑区视网膜厚度与脉络膜厚度的变化(1)
【摘要】 目的:研究儿童先天性白内障术后黄斑区视网膜厚度变化,并分析与术后黄斑中心凹下脉络膜厚度的相关性。方法:回顾性分析42例在本院行先天性白内障手术患儿的临床资料,按年龄分成三组:3~6岁21眼,7~10岁23眼,11~18岁18眼。对术后黄斑区视网膜厚度(MRT)、黄斑中心凹下脉络膜厚度(SFCT)等相关指标进行测量分析。结果:三组患儿术后MRT分别为(239.29±35.54)、(254.83±26.42)、(219.50±16.53)μm,均高于术前,差异均有统计学意义(P<0.05)。术后MRT与SFCT呈正相关(P<0.05)。单眼患儿与双眼患儿的术后MRT比较,差异无统计学意义(P>0.05)。单眼患儿与双眼患儿术后SFCT比较,差异无统计学意义(P>0.05)。结论:儿童先天性白内障术后黄斑区视网膜厚度较术前增厚,与黄斑中心凹下脉络膜厚度呈正相关,单眼患儿和双眼患儿者术后黄斑区视网膜厚度无差异。

[Abstract] Objective: To quantitively evaluate the changes of macular retinal thickness after congenital cataract surgery and analyze the correlation with subfoveal choroidal thickness. Method: Retrospective analysis of the clinical data of 42 children undergoing congenital cataract surgery in our hospital, they were divided into three groups according to the age: 3-6 years old 21 eyes, 7-10 years old 23 eyes, 11-18 years old 18 eyes. The correlation indexes such as retinal thickness (MRT) in macular region and central concave choroid thickness (SFCT) were compared. Result: The postoperative MRT was (239.29±35.54)μm, (254.83±26.42)μm, (219.50±16.53)μm in the three groups, higher than those before surgery, the differences were statistically significant (P>0.05). Postoperative MRT was positively correlated with SFCT (P>0.05). There was no significant difference in postoperative MRT between monocular and binocular patients (P<0.05). There was no significant difference in postoperative SFCT between monocular and binocular patients (P<0.05). Conclusion: Macular retinal thickness after congenital cataract surgery is thicker than that before surgery, the subfoveal choroidal thickness is positively correlated with the macular retinal thickness, there is no difference between monocular and binocular subfoveal choroidal thickness after surgery.

先天性白内障是儿童致盲性眼病之一,多数与基因有关[1],手术是主要的治疗方法。由于婴儿在出生时眼球各组织结构未完全发育,调节、融合、辐辏等功能也尚未成熟,术后可能会发生较多并发症如:弱视、斜视、黄斑水肿等,最终导致视力永久低下。大多数学者认为与术后视网膜厚度改变有关[2],随着光学相干断层扫描技术的发展,有学者发现术后脉络膜厚度也发生了改变,并且对视力影响显著[3]。但上述研究大都针对成人,对儿童的研究较少,为了进一步探讨先天性白内障患儿术后影响视功能恢复的原因,为临床治疗提供依据,笔者对42例先天性白内障患兒进行回顾性研究,分析术后视网膜厚度的变化。由于脉络膜与视网膜联系密切[4],本文增加了对术后两者厚度相关性的分析,现报道如下。, 百拇医药(张琳琳 赵军 张娟美 房祥杰)
