膝关节镜下切开复位内固定对胫骨平台骨折患者术后膝关节功能恢复的影响(1)
【摘要】 目的：觀察膝关节镜下切开复位内固定对胫骨平台骨折患者术后膝关节功能恢复的影响。方法：选择2017年1月-2019年1月于本院治疗的82例胫骨平台骨折患者为研究对象。按照随机数字表法将其分为观察组和对照组，每组41例。对照组接受传统切开复位内固定术，观察组接受膝关节镜下切开复位内固定术。比较两组围术期指标、手术效果、膝关节功能评分及并发症发生情况。结果：观察组切口长度、住院时间均短于对照组，VAS评分低于对照组，差异均有统计学意义(P<0.05)。观察组治疗优良率为95.12%，虽高于对照组的90.24%，但两组比较，差异无统计学意义(P>0.05)。术后，两组Rasmussen评分均高于术前，差异均有统计学意义(P<0.05)。观察组并发症发生率为4.88%，低于对照组的19.51%，差异有统计学意义(P<0.05)。结论：胫骨平台骨折患者接受膝关节镜下切开复位内固定术治疗是安全可行的，可缩短住院时间，切口小且并发症少，利于减轻患者疼痛感，恢复膝关节功能。

[Abstract] Objective： To observe the effect of knee arthroscopic open reduction and internal fixation on the recovery of knee joint function in patients with tibial plateau fracture after operation. Method： A total of 82 cases of tibial plateau fracture treated in our hospital from January 2017 to January 2019 were selected as the study objects. According to the random number table， they were divided into observation group and control group， 41 cases in each group. The control group received traditional open reduction and internal fixation， the observation group received arthroscopic open reduction and internal fixation. The perioperative indexes， surgical effects， knee function scores and complications were compared between the two groups. Result： The incision length and hospitalization time of the observation group were shorter than those of the control group， VAS score was lower than that of the control group， the differences were statistically significant (P<0.05). The excellent rate of treatment in the observation group was 95.12%， which was higher than 90.24% in the control group， but there was no significant difference between the two groups (P>0.05). After operation， Rasmussen score of the two groups was higher than that before operation， the difference was statistically significant (P<0.05). The incidence of complications in the observation group was 4.88%， lower than 19.51% in the control group， the difference was statistically significant (P<0.05). Conclusion： It is safe and feasible for patients with tibial plateau fracture to receive open reduction and internal fixation under knee arthroscope， which can shorten the hospital stay， the incision is small and the complications are few， which is beneficial to reduce the pain of patients and restore the function of knee joint.
