微创经皮椎弓根螺钉内固定手术对脊柱骨折患者后凸Cobb角及疼痛程度的影响(1)

     【摘要】 目的:探讨微创经皮椎弓根螺钉内固定手术对脊柱骨折患者后凸Cobb角及疼痛程度的影响。方法:选取本院2016年1月-2019年12月收治的110例脊柱骨折患者,依据手术方法不同分为对照组与研究组,各55例。对照组行开放椎弓根螺钉内固定术治疗,研究组行微创经皮椎弓根螺钉内固定手术治疗,比较两组手术前后影像学参数,疼痛程度、疗效及术后生存质量。结果:术后,研究组后凸Cobb角低于对照组(P<0.05)。研究组治疗有效率高于对照组(P<0.05)。术后,研究组视觉模拟评分法(visual analogue score,VAS)评分低于对照组(P<0.05),各维度生存质量评分均高于对照组(P<0.05)。结论:采用微创经皮椎弓根螺钉内固定手术治疗脊柱骨折效果理想,可有效改善患者后凸Cobb角,生存质量及疼痛程度,值得在临床中推广。

    Effects of Minimally Invasive Percutaneous Pedicle Screws Osteosynthesis on Kyphosis Cobb Angle and Pain Degree in Patients with Spinal Fractures/YANG Zhongli, LIU Feng. //Medical Innovation of China, 2020, 17(22): 0-016

    [Abstract] Objective: To explore the effect of minimally invasive percutaneous pedicle screw osteosynthesis on kyphosis Cobb angle and pain degree in patients with spinal fractures. Method: A total of 110 patients with spinal fractures admitted to our hospital from January 2016 to December 2019 were selected. They were divided into control group and study group according to different surgical methods, with 55 patients in each group. The control group was treated with open pedicle screw internal fixation, while the study group was treated with minimally invasive percutaneous pedicle screw osteosynthesis. Imaging parameters, pain degree before and after surgery, efficacy and postoperative quality of life were compared between the two groups. Result: After surgery, kyphosis Cobb angle in the study group was lower than that in the control group (P<0.05). The effective rate of the study group was higher than that of the control group (P<0.05). After surgery, visual analogue score (VAS) score of the study group was lower than that of the control group (P<0.05). The postoperative quality of life scores of all dimensions in the study group were higher than those in the control group (P<0.05). Conclusion: Minimally invasive percutaneous pedicle screws osteosynthesis for the treatment of spinal fractures has an ideal effect, which can effectively improve kyphotic Cobb angle, quality of life and pain degree of patients, and is worthy of promotion in clinical practice.

    脊柱骨折屬于骨科常见疾病的一种,临床多以胸腰段为主,约占50%。经皮椎弓根螺钉内固定手术主要将椎弓根钉置于患者的肌肉深处,而后再将骨折脊柱进行复位并行固定[1]。在初期阶段,经皮椎弓根钉手术还不是很成熟,容易造成病灶部位疼痛(或皮肤破损)。随着医疗技术的不断进步,可以先通过透视技术对脊行详细的确认,而后再行椎弓根钉置入,以此来减少术中切口,使得患者疼痛情况得到有效缓解。常规开放手术因治疗痛苦大,术后易出现并发症,因此临床采用不广泛[2]。而微创经皮椎弓根螺钉内固定手术对关节(或神经组织)损伤较小,因此,更易被患者接受。本研究主要探究微创经皮椎弓根螺钉内固定手术对脊柱骨折患者后凸Cobb角及疼痛程度的影响,现报道如下。, 百拇医药(杨忠利 刘枫)
