自然分娩专科护理项目干预对初产妇自然分娩质量、产后并发症预防的影响(1)
【摘要】 目的：探讨自然分娩专科护理项目干预对初产妇自然分娩质量、产后并发症预防的影响。方法：选取2019年1-12月本院拟自然分娩的初产妇140例为研究对象，按随机数字表法分为干预组和对照组，各70例。对照组给予产科常规护理，干预组给予自然分娩专科护理项目干预。比较两组分娩情况、各产程时间、产后并发症、分娩知识、喂养知识以及护理满意情况。结果：干预组会阴侧切、阴道助产、产程延长及新生儿窒息率均低于对照组(P<0.05)。干预组第一、二产程时间均短于对照组(P<0.05)。干预组产后并发症发生率为14.29%，低于对照组的28.57%(P<0.05)。干预组分娩与喂养知识评分均高于对照组(P<0.05)。干预组护理总满意率为95.71%，高于对照组的85.71%(P<0.05)。结论：自然分娩专科护理干预有助于提高初产妇自然分娩质量，降低产程延长发生，预防产后并发症，提高母婴安全，值得推广。

[Abstract] Objective： To explore effects of natural delivery specialist care intervention on quality of natural delivery and prevention of postpartum complications in primiparas. Method： A total of 140 primiparas planned to delivery naturally in our hospital from January to December 2019 were selected as the research objects. They were divided into intervention group and control group according to the random number table method， 70 cases in each group. The control group was given routine obstetric care， while the intervention group was given natural delivery specialist care intervention. The delivery situation， the time of each stage of labor， postpartum complications， delivery knowledge， feeding knowledge and nursing satisfaction were compared between the two groups. Result： The rate of episiotomy， vaginal midwifery， prolonged labor and neonatal asphyxia in the intervention group were lower than those in the control group (P<0.05). The first and second stage of labor in the intervention group were lower than those in the control group (P<0.05). The incidence of postpartum complications in the intervention group was 14.29%， it was lower than 28.57% in the control group (P<0.05). The scores of delivery and feeding knowledge in the intervention group were higher than those in the control group (P<0.05). The total nursing satisfaction rate of the intervention group was 95.71% higher than 85.71% of the control group (P<0.05). Conclusion： The natural delivery specialist care intervention can help to improve the quality of natural delivery for primipara， reduce the occurrence of prolonged labor， prevent postpartum complications， and improve the safety of mother and child.

初產妇由于缺乏分娩经验和医学知识，对于自然分娩产生紧张、担忧等负性情绪，影响自然分娩质量，增加产后并发症[1-2]。为满足自然分娩产妇需求，开展优质护理服务，规范自然分娩专科护理临床路径管理成为我国产科医疗管理的热点，有利于降低住院日，节省住院费用，提高产妇满意率，并增进医患沟通[3-4]。国内对于自然分娩专科护理临床路径管理尚处于起步阶段，发展并不成熟。自然分娩护理项目多且繁杂，产科护士人力资源紧张，在一定程度上影响产妇自然分娩质量，不利于优质护理服务的开展[5-6]。本院产科在查阅相关文献、查阅院内产科病历、临床调查与访谈、Delphi法专家咨询的基础上，制定l详细的自然分娩专科护理项目，按照此实施临床路径管理，保证产妇自然分娩专科护理项目的标准化、规范化、精细化，规范临床操作，进行自然分娩风险控制，强化医疗管理，提高医院资源的管理和利用，取得了满意效果，现报道如下。, 百拇医药(卜香兰)
